Anterior vs Posterior Circulation
- The brain gets anterior supply from the carotids and posterior from the vertebrals forming the basilar artery.
- Vascular anatomy is variable so focus on where vessels end and branch rather than simple directions.
Remember Four Main ICA Branches
- Memorize the classic four internal carotid branches: ophthalmic, PCOM, anterior cerebral, and middle cerebral.
- Use the mnemonic OPAM to recall Ophthalmic, PCOM, ACA, MCA quickly on exams.
ACA Stroke Clinical Pattern
- ACA strokes classically produce more leg weakness than arm weakness and may cause abulia or urinary incontinence.
- Think superior frontal/superomedial cortical strip when you see those features.
