There is the modified Boston criteria that can help guide or diagnosis based on imaging and symptomatology. The only definitive way to diagnose cerebral amyloid angiopathy is with tissue sampling. For a definitive diagnosis, a brain biopsy is needed per these criteria. There are other vasculopathy that we should think about, for example, vessel dissection.
