Video laryngoscopy showed higher first-attempt intubation success rates than direct laryngoscopy, supporting its effectiveness in critical care.
Prehospital TXA use in severe trauma patients did not result in significant neurological outcome differences compared to a placebo, suggesting further investigation is needed.
Deep dives
Comparison of Video and Direct Laryngoscopy for Tracheal Intubation in Critically Ill Adults
A multicenter RCT compared video versus direct laryngoscopy for tracheal intubation in critically ill adults. The study found that video laryngoscopy resulted in a significantly higher rate of successful intubation on the first attempt compared to direct laryngoscopy. Patients in the video laryngoscopy group had an 85% success rate, while the direct laryngoscopy group had a 71% success rate. The study highlighted the importance of selecting the appropriate device for tracheal intubation to enhance patient outcomes.
Pre-hospital Tranexamic Acid for Severe Trauma Functional Outcome Study
The PATCH Trauma trial examined the use of pre-hospital tranexamic acid in severe trauma patients to assess functional outcomes. This international double-blind randomized trial involved over 1,300 patients across multiple ambulance services and hospitals. The study focused on patients over 18 with suspected severe traumatic injuries deemed high risk for trauma-induced coagulopathy. Results showed no significant difference in favorable neurological outcomes between patients receiving tranexamic acid and those receiving a placebo, indicating a need for further research.
Intensive Inpatient Blood Pressure Management in Older Adults
A retrospective cohort study explored the impact of intensive blood pressure management in hospitalized older adults. The study, encompassing over 66,000 patients, revealed that one in five patients who received intensive treatment experienced adverse clinical outcomes. Patients receiving intravenous anti-hypertensives exhibited higher risks. The findings suggested that acutely treating asymptomatic inpatient hypertension could lead to harmful effects, emphasizing the importance of carefully evaluating the need for blood pressure interventions in hospitalized individuals.
Conclusion and Future Implications
The podcast episode discussed three significant papers encompassing tracheal intubation techniques, tranexamic acid use in trauma, and blood pressure management in hospitalized patients. The studies shed light on the complexities of medical interventions and highlighted the critical importance of evidence-based practices. The need for further research to refine treatment approaches and improve patient outcomes was underscored throughout the discussions.
There have been some huge trials released over the last month and we've got three brilliant papers to discuss!
First up we take a look at an RCT on video versus direct laryngoscopy for patients requiring emergency intubation with the DEVICE trial. The VL versus DL debate has been ongoing for quite some time now, so is this a final nail in the coffin for DL?
Next up we take a look at an RCT of prehospital TXA use in patients at risk of bleeding from major trauma in the PATCH trial. The results seen in the trial look at a glance to oppose those seen in CRASH-2, so is this the end of TXA in this cohort of patients?
Finally we have a great paper giving us further information on whether we should we be initiating immediate antihypertensive treatment for patients admitted to hospital with asymptomatic hypertension.
Once again we’d love to hear any thoughts or feedback either on the website or via twitter @TheResusRoom.
