Dr. Courtney Conley on Assessing, Treating and Training the Foot

As children, most of us are born into the world with two happy and healthy feet. They move, bend, and push just like they were designed to. But over the years, whether it’s through bad shoes, lack of motion, or flat out weakness, we tend to lose those ideal foot mechanics – and the downside can mean a negative cascade of events throughout the body. As someone that not only had foot issues herself, Dr. Courtney Conley was always interested in feet and how to get herself out of pain. This led her down a path of not only chiropractic school but also working in an orthotic lab early in her career. So how do you imagine she felt when she came to the stark realization that the product she was creating was actually BLOCKING normal foot function??? That realization, combined with an insatiable thirst for knowledge on everything feet, brought her to where she is today. Dr. Courtney Conley is the founder and creator of Gait Happens where she has the opportunity to truly realize her dream of helping as many people as possible reclaim their foot function. She’s an internationally renowned foot and gait specialist who teaches both nationally and internationally. Dr. Conley is a chiropractic physician who specializes in foot and gait mechanics. She holds a BA in Kinesiology, a BA in Human Biology and a doctorate in Chiropractic Medicine. But most importantly, Dr. Courtney knows FEET – and that’s what we’re going to talk about here today!   My heart is so in this because when people can’t walk because their foot hurts, it’s brutal. It’s painful, and it hurts me. Because I know what it does for my mental health and my mental capacity, and still does every day. So I don’t think we need to make it any harder than it is, we just need to get out and walk more. – Dr. Courtney Conley   This Week on the Physical Prep Podcast: What originally got Dr. Courtney into the world of physical preparation, and most importantly, why she’s so interest in FEET. The role the foot places on performance as a whole. Foundational gait mechanics, and what your foot SHOULD be doing throughout the gait cycle. The differences you see when moving from walking to running mechanics. “High Gear”: What it is, why it’s important, and ways your clients and athletes will try to cheat it. Why people shouldn’t be called “Pronators” or “Supinators.” Toe gripping, foot strength, and why your foot is so important in everyday movement. Dr. Conley’s assessment process, as well as self-assessments you can do with yourself or your clients/athletes. The role tibial internal rotation (IR) plays in effective gate. Courtney’s thoughts on barefoot training and the minimalist shoe phase. Last but not least, Dr. Courtney’s shoe recommendations, and the two things she looks for in every pair of shoes she buys or recommends.   And last but not least, I’m trying the video thing here as well – especially since Courtney had some visuals in there as well. 10 Programming Tips I Wish I Would’ve Known Starting Out

When you’re just starting out as a trainer or coach, as the saying goes: You don’t know what you don’t know. And I think this is especially true with regards to writing programs. Whether it’s using antiquated ways to program intensity, making everyone conform to 4-week training cycles, or skipping key steps early-on in the training process, today I want to share with you 10 things I wish I would’ve known when I first started writing programs. I’ve got a blend of both big-picture, philosophical ideas and practical, ready to use tips thrown into the mix, so I really hope you’ll find this episode useful!   When you’re writing a program you have to start with the long-term goal(s) first, and then determine the KPI’s from there. – MR   This Week on the Physical Prep Podcast: What length of training block is best? And should everyone be rotating exercises every 4-weeks? Why building training blocks of 1-RM’s isn’t the best way to program intensity, and alternatives you can use instead. Thoughts on building a solid training base that will last you for months (or years) to come. Why I’m working to be more well-rounded in every program I write. The client psychology you MUST be aware of when writing programs. Why Bill Hartman is such a big believer in “immediate wins,” and how it helps you improve buy-in. KPI’s: What should you be tracking? What metrics are important? And how do you maintain or improve buy-in by tracking multiple things? Should you focus on emphasizing strengths or eliminating weaknesses? Making the variables of a training program congruent for optimal results. How you can use tempo to improve the efficacy of a training program. Using tempo to target muscles versus connective tissues in training. Why fun is such a critical factor to your – and your clients’ – success.   Stop the Frustration! We’rrrreeeee back! It’s the February edition of our Q&A podcast, and man do we have a bunch of topics to cover. Performance assessments. Isometric training and testing. Why everyone and their sister is squatting with a heel wedge. And why IFAST is evolving. So without any further ado, let’s jump right in!   Isometrics are one part of a program. A powerful tool? Yes. But if they’re the only tool? That’s a problem. – Mike Robertson   This Week on the Physical Prep Podcast: Team Robertson life update If I got a well-moving and injury-free athlete, what five performance tests would I use to evaluate them? My journey back to competitive soccer, and the NUMEROUS lessons I learned along the way How to train clients/athletes who are always getting injured Why we see heel/squat wedges in every squat variation everywhere on the Internet Where I put ISO’s in my testing/training, and thoughts on how to get the most out of them The Big Question for the Month: Why we’re stopping the IFAST internship program   Related Links: Marc Bubbs Podcast Alex Natera Podcast Drake Berberet Podcast Jarrod Antflick Podcast Why Does Elevating My Heels Make My Squat Look Better? Aaron Cunanan on Developing a Sports Science Team in Pro Baseball

One of my favorite aspects of this show is getting these little peeks behind the “high performance” curtain. Even if we don’t work in elite sport, or have the resources they have available to them, getting an understanding into the systems and thought processes they use to build the best athletes in the world can still be invaluable. Plus, if I get to talk to a former IFAST intern who is absolutely crushing it in life, that’s never a bad thing, either! Which is why I’m absolutely thrilled to have my guy Aaron Cunanan back on the Physical Preparation Podcast. Aaron is entering his second season as the Director of Applied Sports Science for the Cincinnati Reds. He has previous stints as Sport Science Coordinator for the San Francisco Giants and Performance Support Manager for the UFC Performance Institute. Aaron’s research background includes athlete monitoring, periodization, and weightlifting biomechanics. And today, we’re going to dive into how he’s creating an athlete-centric approach to sports science with the Cincinnati Reds!   One of the biggest challenges to baseball is it has the most grueling competition schedule in professional sports…and the game doesn’t care if you’re ready to play 162 games or not. – Aaron Cunanan   This Week on the Physical Prep Podcast: Aaron’s long and winding career path through Olympic weightlifting, professional baseball, and the UFC. What it means to be people-driven versus data driven. How a sports-science truly becomes athlete centric, versus just using that language to sound cool. Aaron’s thoughts on how data and sports scientists can best serve the athletes and coaches they’re working with. The advice he’d give to someone who is new to a sport – especially since he had about zero experience in baseball or MMA when he started there! The key performance indicators (KPI’s) he’s found to be relevant in baseball. His thoughts on finding natural ways to create engagement and build rapport with coaches and athletes. The advice he’d give to someone who is new to the sports science realm, and wants to learn more without getting overwhelmed! Aaron’s 10-15 year ideal plan for the Cincinnati Reds, and the role he’d like sports science to play in that process.   Connect with Aaron: Aaron Cunanan on Twitter   Related Links: Aaron Cunanan on Sports Science, Athlete Monitoring and Being Evidence Driven   Stop the Frustration! Dan Pope on Training, Injuries, and How to Stay in the Gym for Life

Let’s be real for a minute: Injuries suck. If you’ve been lifting yourself for any period of time, you know that if you push yourself long enough or hard enough, something is bound to crop up. But while injuries might be part of the game, they shouldn’t keep you out of the game. That’s why today I’ve brought Dan Pope on the podcast – to talk about how we can continue to train while injured, but even more importantly, how we can learn from our injuries as well. Dan is a Doctor of Physical Therapy, strength and conditioning coach, writer, speaker and entrepreneur. He’s a practicing physical therapist at Champion PT and Performance, and owner of the FitnessPainFree.com website and certification. Dan’s mission is to bridge the gap between physical therapy and fitness, and to bring this information to those around the world, and I really think you’re going to love the practical advice he shares in this episode!   I want them to have a little mindfulness about their pain, because if you’re just blowing through it, that’s not helpful and pain WILL win. – Dan Pope   This Week on the Physical Prep Podcast: Dan’s career path, and how he went from strength coach to physical therapist His overarching philosophy on dealing with clients and athletes injuries How your mindset has to shift when dealing with an injury Dan’s insights into the two types of rehab clients/patients you’ll see, and why you need to treat them VERY differently The potential causes of lower back pain, and why mechanics are just one part of the problem Tweaks and modifications you can use to work around lower back pain and stay in the gym His thoughts on anterior knee/patello-femoral pain Why “joint sparing” training strategies often rob Peter to pay Paul How Dan helps clients and athletes learn from their injuries so they can stay in the gym for a lifetime   Connect with Dan: FitnessPainFree.com Dan’s YouTube Page Dan’s Instagram   Related Links: Stuart McGill on Building the Athletic Spine Mike Reinold on the Science & Practice of Blood Flow Restriction Training Jarrod Antflick on Building a Resilient and Robust Achilles Tendon $100M Offer – Alex Hormozi   Stop the Frustration! Jarrod Antflick on Building a Resilient and Robust Achilles Tendon

Have you ever had a client or athlete who was dealing with Achilles pain? Maybe it was a gen pop client who was a little overweight. Maybe it was a weekend warrior whose heel was stiff after a game of basketball or pickleball. Or maybe it was a high performing athlete who was dealing with tendinopathy or rehabbing an Achilles tear. Regardless, Achilles issues are on the rise and as trainers and coaches the more informed we are about these issues, the better we can take care of the clients and athletes we’re working with. That’s why today I’ve brought on the “James Bond of Tendinopathy” – Jarrod Antflick. Jarrod consults to teams in the UK’s English Premier League, Premiership Rugby, tennis, track and field, as well as the NBA and NFL teams in the USA. He has a Masters in Sports Physiotherapy from La Trobe University, and is currently a PhD candidate at Imperial College, London. He also works as consultant physiotherapist at The Fortius Clinic London, as a specialist in tendinopathy undertaking research in the management and diagnosis of tendinopathy.   If you said to any strength coach “You’re only allowed to do isometric training” they’d probably drop their lifting shoes and walk out the door. They’re going to want to do concentric and eccentric training because that’s how sport is played. – Jarrod Antflick   This Week on the Physical Prep Podcast: How Jarrod got started in the world of physical preparation. Lifestyle factors that play a critical role in the health of the Achilles tendon. The similarities – and differences – between the Achilles tendon and the patellar tendon. Common areas of injury for the Achilles tendon. Diagnosing Achilles tendon issues: From simple tests trainers and coaches to use, to high-end imaging and diagnostics. Does imaging correlate to pain or dysfunction in the literature? The role of isometrics (ISO’s) in managing tendinopathy. Are they really all they’re cracked up to be? Why the S&C or physical prep coach plays such a pivotal role in keeping Achilles tendon issues at bay. How the old-school periodized program can help manage and treat Achilles issues. Thoughts on different calf exercises (seated, standing, standing with knee bent, etc.) and the role they play in the training program. Steve Tashjian and Darcy Norman on Building a High-Performance Team at the International Level

The World Cup is the pinnacle of the soccer world. You have the best athletes in the world, representing their countries, and doing everything they can to perform at the highest levels in the biggest moments of sport. And while I love nothing more than watching these incredible athletes play, I also love talking to the coaches BEHIND those athletes – the people that help support these amazing athletes in their quest for soccer immortality. And that’s why I’m so honored to have two members of the US Men’s National Team Performance staff on today’s episode – Steve Tashjian, Head of Performance during the last World Cup cycle, and performance coach Darcy Norman. Steve Tashjian was a soccer hack that got a taste of what bad rehab looked like and wanted to make things better for the athletes he worked with. He started his coaching career in the MLS with the Columbus Crew, then spent 5 years working in England at Everton Football Club, before coming back for a second stint with the Columbus Crew. And not to be outdone, Darcy Norman has spent time with prestigious clubs like Bayern Munich and AS Roma, and oh by the way, he also won a World Cup with the German Men’s National team in 2014! So needless to say, this is an awesome episode and one I think you’re going to love!   Once they saw it operating, and how we operated, then everyone was aligned and we were great. The interactions with the clubs from a compliance and data-sharing standpoint was 95%. – Darcy Norman   This Week on the Physical Preparation Podcast: What’s new with Steve and Darcy since I last chatted with them. Reflecting on the World Cup experience as a whole. Selecting the performance staff for the USMNT, and the roles and responsibilities of everyone involved. Thoughts on creating an athlete-centric system where you can have open and vulnerable discussions with top-shelf soccer clubs across the globe. The onboarding process that the athletes were taken through prior to getting started. How you effectively manage (and keep tabs on) 40-50 athletes who are playing games across multiple continents. Developing a fitness/physical preparation program that supports the coach/manager’s preferred style of play. Key Performance Indicators (KPI’s), and how Darcy and Steven went about blending subjective feedback with objective, data-driven tools. Game prep: What do you do to help ensure your athletes are fit, fresh and ready to go on only 3-4 days rest? A final debrief, and what the future holds for both of these awesome coaches!   Connect with Steve: ElevationProX.com Steve on LinkedIn   Connect with Darcy: Darcy on Twitter Darcy on LinkedIn   Related Links: Steve Tashjian on Logistics, Integration and High-Performance Teams Darcy Norman on the Value of Working with Interesting People Darcy Norman Reflects on His First 2 Years with AS Roma Physical Preparation with Darcy Norman   Supercharge Your Decision-Making and Program Design with Hawkin Dynamics Force Plates! Steve Long on Finding Balance & Coming Full Circle in the Fitness Industry

Steve Long has been in the fitness industry for almost 20 years now, and according to him, still feels like a dumbass on a weekly to daily basis when working with clients. This feeling drives him to continually learn more, to be able to help his clients, and share some of the best nuggets that he picks up along the way with other training professionals. Like anyone who’s been in the industry for a while, I too have had my share of ups and downs. Because trust me – nothing will put you on an emotional rollercoaster like owning your own business! So between Steve and I’s shared experiences, I think we’ve got a great show for you here today.   I shut down a large (8,000 square foot) facility where we did a  lot of large group training to focus pre-dominantly on semi-private because that’s what I like to do – I like to dive into the minutiae of training. – Steve Long.   This Week on the Physical Prep Podcast: Steve’s start in the fitness industry, and how strong mentors early-on introduced him to continuing education. Why he became stagnant and bored with ConEd, and why burnout is such a real thing in our industry. What ultimately led him back to ConEd, and why the White Belt or “Beginners Mindset” approach has served him well. How continuing education actually fueled changes in his own training and physique. Why building a bigger, badder gym may not be the best idea – and why Steve’s gone back to an old-school, 1,000 square foot studio. How starting martial arts training has led to a big shift in his business model and ideal client. General training thoughts, including his thoughts on resets and training in a more athletic fashion. And last but not least, arguably the best answer to the “Big Question” that I’ve ever had on this show. It’s worth the price of admission!   Connect with Steve: Steve on YouTube Steve on IG Steve on Facebook   Stop the Frustration! Dr. Paul Comfort on Assessing and Evaluating Force Production

Dr. Comfort is a Professor of Strength and Conditioning and program leader for the Master of Science Strength and Conditioning program at the University of Salford. He is also an adjunct professor at Edith Cowan University in Western Australia. Paul has applied experience across a range of sports and consults with numerous professional and semi-professional sports teams, which he has undertaken throughout his academic career. Last but not least, Paul is a founding member of the UKSCA, is on the NSCA Board of Directors, and is a senior associate editor for the Journal of Strength and Conditioning Research, an associate editor for the European Journal of Sport Science and Sports Biomechanics. So needless to say, I think he knows a thing or two about the topic at hand!   If you’re already squatting 2-2.5x’s body weight, you’re better off spending more time on your ballistic and plyometric capabilities than you are on maximum force production. – Dr. Paul Comfort   This Week on the Physical Preparation Podcast: The major differences between standard, repetition maximum (RM) testing and isometric testing. What isometric testing can give you that RM testing cannot. Best practices for the isometric mid-thigh pull (IMTP) test. The biggest issues Dr. Paul sees with this test, and his “non-negotiables” for set-up and performance. Do you need separate tests to tease out the differences between peak force and rate of force development (RFD)? How do get valid and reliable data from your RFD testing, as well as what time measurements Dr. Paul looks at the most. The differences between an IMTP and isometric squat test, and why you might choose one over the other. Don’t have the funds to invest in force plates? Here’s what you might consider instead. A really fun lightning round where we discuss pet peeves in research, Dr. Comfort’s favorite research to date, and his thoughts on micro-dosing strength training in sport.   Connect with Dr. Paul Comfort: Dr. Paul Comfort on Twitter Dr. Paul Comfort on Instagram   Related Links: Standardization and Methodological Considerations for the Isometric Midthigh Pull Training Force with Dr. Paul Comfort   Supercharge Your Decision-Making and Program Design with Hawkin Dynamics Force Plates!
