Returning to work with Back Pain
In this Q and A episode, Rob and Dave look at a question from their Facebook community about when you should return to work if you have back pain They have great information about graded exposure to practise movement and help the back to adapt before you return to the workplace and they also share great tips on how to balance and vary your movement activities when you are at work so that your body can cope successfully with the demands you are making on it KEY TAKEAWAYS If you first experience back pain at work you can associate it with being the cause of the pain The spine is designed to bend and lift Movement avoidance can result in fear around making any movement Short term avoidance can be beneficial for initial relief but it’s vital to get back to movement as soon as possible Bending is unavoidable as a part of everyday life You have to practise movement unloaded before you do the movement loaded You need to use graded exposure before you return to work or activities – slowly pushing the body and allowing it to adapt to the stresses Work out what the trigger movements are and change your movement to let your body adapt Choose a movement activity from work that you are hesitant about and trial doing it at home before you return to work You need to slowly retrain the body to cope with the demands of work Walking is a good way to start with weight-bearing on the spine BEST MOMENTS ‘Use those joints that’s what they were made for’ ‘You should practise movements’ ‘It’s training your body to cope with the demands you put on it’ ‘Sometimes the fear of movement can lead people to avoid movement’ THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. If Everyone has a Disc Bulge, Why Does Mine Hurt so Much
Rob looks at disc bulges and explains why for some there is little or no pain whilst others experience pain that impacts every aspect of their daily lives. He discusses the key questions that can help in understanding an individual’s experience of pain and explains how slow and subtle changes to the body are perceived very differently from sudden changes, which can be interpreted as a threat and experienced as pain This is a hugely informative episode that will help anyone understand more about their disc bulge, experience of pain and successful recovery KEY TAKEAWAYS The pain can have taken over your daily life Having a discussion with your clinician about imaging findings is a key part of the recovery process A clinician may ask about fears, beliefs and understanding around pain to help them understand more A good opening question is ‘what do you think is the cause of your pain?’ Do you believe this pain will ever go away? The conversation and approach with a patient will vary according to how this question is answered Most people walking around have disc bulges, some have no pain some have lots of pain When load is experienced as too quick, too strong, or too often, the brain can interpret this as a threat and pain can be experienced If the stresses are applied gradually the joints and tissues have the chance to adapt and get stronger BEST MOMENTS ‘People who have been in pain for a long time may believe it will never go away’ ‘There are many factors that impact on someone’s experience of pain ‘Two MRI scans can look identical but one person is in agony and the other has never suffered from back pain’ THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. What does a Chiropractor do when he has Sciatica? - Jacob Harden
This is a patient story with a twist because the patient is practitioner Dr Jacob Harden, a chiropractor based in Florida He shares a unique insight into his journey living with and recovering from sciatica and discusses managing pain and the impact on his mental health. There are lots of great takeaways from this episode with great advice on how to track your progress and get back to daily routines and activities a step at a time, if you have sciatica this is a great reminder that recovery is a journey and reducing provocative behaviours can make a huge difference KEY TAKEAWAYS The neurological symptoms were an indication that this was not going to be a quick fix Any MRI is just one piece in the jigsaw and any scan needs to be carefully explained I’ve got better little by little and overall, I’ve come a long way List all the daily activities you are struggling with and work on achieving them Work with the tolerance of your body at the time to expand your capabilities Regulate on a day by day basis and moderate and adapt when needed Accept that pain is part of the process and that it will improve over time, your role is to control the process Changing your mindset and tracking the small wins can impact positively on recovery Pain reduction may not be a reflection of the progress being made Objective markers and finding a metric outside of pain numbers can really help with understanding how you’ve come Mood affects outcomes and pain can be the catalyst for emotional distress Pain is part of it and understanding how it’s impacting on living your life can help with moving forward Take it step by step if you are progressing, you are moving in the right direction, it’s an evolving process BEST MOMENTS ‘Every time I shifted from one side to another there was pain’ ‘Self-reflection and recognising where you are and how you are feeling about the pain can help in moving forward’ ‘An MRI is just a single snapshot of a moment in time’ ‘Take time to talk to others about how you are feeling’ THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ RESOURCES FOR THIS EPISODE Dr Jacob Harden Facebook Dr Jacob Harden Instagram ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Does Form Matter When Lifting - Andrew Lock VS Greg Lehman
In this episode, Jared Powell host of ‘On the Shoulders of Giants’ YouTube channel oversees a fascinating discussion with Greg Lehman and Andrew Lock who are debating lifting techniques and lower back pain Both hold strong opinions and share great insights into their personal position and why they hold the opinions they do in this insightful and absorbing discussion Whether you are lifting a box or lifting weights this is a discussion worth listening to before you make up your own mind. KEY TAKEAWAYS The research done to date doesn’t really answer the question Very few injuries are the result of a single event, it’s accumulated micro trauma Movement is neurological skill acquisition and rehabilitation is teaching the skill to move efficiently and effectively There are a set of noble principles with variables that can be used to produce a safe lifting technique When we lift we are using we are using our body levers to move load and doing it as efficiently as possible We have bodies that vary and principles need to be applied to the individual, strength is specific and lifting is specific to the task All coaches say they work on best technique according to the individual We can all recognise that we are different and can look at the research and come to different conclusions Lifting is about the hip axis and you need to maximise the torque Get into the position where the hips are in the primary focus When people compare different spine models there is an issue because the biomechanics disagree You have to earn the right to change the position Transference of movement beyond the gym is an important aspect of biomechanical function Strength is a skill and skill comes from practise, perfect practise produces perfect skills that can be transferred into daily life Biomechanical injury is not random, it’s observable, it’s assessable and predictable Physics governs us all You need to learn to move in the best fit pattern for your body BEST MOMENTS ‘This debate has never been settled’ ‘There cannot be one lifting position it’s preposterous’ ‘Spinal flexion is a beautiful thing ‘That there is only one best lifting position is said by no one’ THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ RESOURCES FOR THIS EPISODE Find Greg on Twitter and Instagram @greglehman Find Andrew on Instagram @andrew_lock_strength Link to the debate on YouTube https://www.youtube.com/watch?v=ciCDko6JGaQ Jared Powell - Shoulder of Giants YouTube Channel https://www.youtube.com/channel/UC9WC27yg4Y8gAwLI64cilug ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. How I recovered from my Sciatica - Paul Babai & Dr Oliver Thomson
Rob and Dave are joined by Dr Ollie Thomson and one of his patients, Paul, who shares his experience of complex back pain and his journey to recovery. Following back surgery, Paul experienced chronic pain resulting in him seeing a number of different practitioners in his quest to successfully recover but as he explains in this fascinating episode it was working with, Dr Ollie Thomson, a professional who helped him to understand the mental aspect of his pain that changed his mind set and finally put him on the road to successful recovery. KEY TAKEAWAYS Persistent pain can become part of your identity and this is especially true of back pain Everything comes from your back and you think if your back is strong you are strong I was looking for a magic fix because I felt there was something fundamentally wrong As a professional, using movement to explore fears, beliefs and threats means you are in a better position to create positive new habits with the patient I needed to be in the right place to understand that I needed to work at it to successfully recover It felt like I had a team that made me accountable but also cared and thought I would get better Thinking about it differently and being focused is what got me out of the situation and a shift in my mindset played a huge part in my recovery You can be very low and feel considerable pain but you can come back from it It’s a constellation of experiences and factors that create someone’s pain experience and situation Overly medicalising back pain and giving it strong anatomical connotations makes people more fearful Trying to make sense of backpain through autonomy and structure can change how people behave It’s about how practitioners communicate with patients to understand their pain experience It’s vital to take the time to listen and speak with the patient, taking a therapeutic approach It’s the role of the professional to guide patients to being able to self-manage their pain and their recovery themselves BEST MOMENTS ‘Providing positive explanations can help with looking at things from a different perspective ‘The mental aspect of having a back issue was really difficult to deal with ‘I was doing a lot of nothing because my belief was that my back was weak because of the pain’ THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCESBook to see Dr Oliver Thomson! Poor Back Pain Advice
Rob and Dave are talking with Rebecca host of the ‘Better Doctor Project’ podcast about misinformation and how getting the right information can be life-changing They discuss some of the misinformation they encounter most frequently and how to move forward from it Pain equals damage and the human body being fragile are common misconceptions that can result in people stopping activities and movement but Dave, Rob and Rebecca share great advice and content about the information you should be listening to in this informative episode KEY TAKEAWAYS The most misleading information is that pain equals damage but this is not true Because people are worried they stop moving You can have immense pain and little damage and immense damage with very little or no pain Pain is a warning sign but doesn’t mean there is damage Its misinformation that the human body is very fragile but we should continue to move and develop strength It’s a misconception that during pregnancy you should stop exercising. Exercising during pregnancy is a positive People seek out the narrative to support their bias, whatever that may be It’s important that professionals keep their knowledge and continuing professional development updated You can never attack or degrade another practitioner when you speak with a client you are getting the information third or fourth hand Your approach may be different to the previous practitioner but it’s vital the client is on board You need to meet the client where they are at currently and never devalue any previous experience Add in good advice and treatment instead of starting again BEST MOMENTS ‘Society has led us to have preconceived misconceptions’ ‘Just because you have thirty years’ experience doesn’t mean you have something better to offer than the young practitioner’ THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ RESOURCES FOR THIS EPISODE Better Doctor Project Podcast ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Is Lifting Bad for My Back?
Dave Skolnik, Doctor of Physical Therapy and co-host of ‘The Essential Strength’ podcast discusses if lifting could actually be good for our backs and how to build resilience As Dave says in reality “lifting is a non-optional part of the human experience so you might as well get good at it” and in this episode, he shares how you can use exercise to prepare your body for life both physically and mentally and how to take the right steps to becoming more healthy Get started with purposeful movement and you will be on track to discover what strength means for you and how you can build functional reserve and a healthier life KEY TAKEAWAYS Lifting within your personal strength is not bad for your back People see lifting as an optional activity but lifting things is an inescapable part of human life Exposing yourself to strength training reduces the risk of injuring yourself in other areas of life When you have grade exposure you build your functional reserve, your res Most of the time people hurt themselves on their maximal effort point, they surpass what they are capable of For each individual person, there is an anatomically ideal way for them to perform a lift Finding your optimal form and building strength will help to reduce the risk of injury Strength training in multiple planes of motion is overlooked but can make a huge difference Exposure to lateral movement is important There is increasing research about the protective benefits of physical fitness and the neuroprotective benefits of resistance training to achieve optimal ageing The person who is twisting, stretching, bending, and flexing is at less risk than the sedentary person who is sitting as their pelvic floor weakens It’s not bending over that is harmful, it’s bending over when you are not prepared If you want to get started with moving find a good coach With the right intention, you can use your own body weight to effectively train at home Reduce the barriers by starting with small movements that you can then build-up BEST MOMENTS ‘Lifting weights is optional, lifting things is obligatory you are going to have to bend over and pick something up at some point’ ‘Bending and lifting and twisting is what the back is designed to do’ ‘Now is the time to start exercising, create some early stage, not perfect but done movement and you are exercising’ THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ Dr David Skolnik Instagram The Essential Strength Podcast Stronger in Motion website ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Ask Us Anything - The Sex and Back-Pain Edition
Dave and Rob answer questions from their communities and in this episode look at what it means when your practitioner says, ‘It’s all in your head’ and share great advice on how pain works and the individual response to it If you are concerned about your posture and think it’s contributing to your back pain will a posture correction device help? Dave has great advice and a clear explanation of what you can do to positively influence back pain They also answer a frequently asked question about sciatica and sex, listen in and find out more KEY TAKEAWAYS It’s the brain’s job to decide if something is painful or not and technically all pain is in your head Your pain is real for you and it’s important that it’s validated You can’t fail physiotherapy or other professions it just means you need to look for a different approach Movement is a positive influence on back pain, movement is key from a neurological, physiological, and chemical point of view Gentle movement is what makes the difference It’s normal to have symptoms after surgery because the nerves still need to heal When a nerve is irritated it changes its state so it takes time to return to being healthy Nerve healing is different for every individual but diet and systemic health play an important part in optimum recovery Your interpretation and reaction to pain will influence your healing time If you can take a ‘top down’ approach and view yourself then you can influence your healing Sciatica does not mean you can’t have sex there it’s about whatever is most comfortable There is no particular position, it’s about finding the position that works best for you Any sexual dysfunction can be a sign of cauda equina and you should seek medical help BEST MOMENTS ‘Freedom to fidget – you should be moving as much as possible ‘Nerves take time to return to their resilient structure’ ‘Everyone has different rates of healing ‘Grab a Karma Sutra and work your way through’ THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. What to Expect from a Good Physiotherapy Consultation
This is the third instalment of the mini-series covering everything Rob and Dave think everyone should know about back pain and looks at what you should expect from a good consultation. Adam Dobson, specialist physiotherapist discusses the five things you should expect from any good consultation from being listened to through to a comprehensive recap. There is lots of great advice to take away and make great practical use of if you are attending a consultation so this is a must-listen KEY TAKEAWAYS The clinician should invite you to tell you your story- they will want to understand what’s important to you, the impact of your problem and what your current understanding is The clinician should create a space where you can feel listened to You should be asked what your expectations are and through discussion, goals should be established and agreed The patient and the clinician should be on the same page When someone has a belief about something for a long time this has to be recognised The conversation should be sufficiently challenging but not make you feel uncomfortable A lot of information is gained from taking a detailed history The clinician will want to examine the body part with the issue and also undertake a movement examination There may also be specific tests relevant to particular tissues and nerves The clinician should do a recap including the nature of the problem, why it’s hurting and biologizing what’s going on It’s about the collaboration between the knowledge of the patient and the knowledge of the clinician BEST MOMENTS ‘A good clinician should be a good listener’ ‘You should be developing a rapport and be on the same page as your clinician’ ‘A good clinician will explain any findings as they go along’ THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ RESOURCES FOR THIS EPISODE Patrick Carroll blog: oldquaterphysio.ie Adam Twitter: adamdobson123.
