Fear Avoidance Behaviour - What to do when a particular movement hurts
Rob goes solo in this episode and is talking about fear avoidance behaviour – when you avoid certain movements due to pain and how it can be overcome. He discusses how it’s possible for fear-avoidance behaviours over an extended period of time can ultimately lead to increased sensitivity and pain and how to break the cycle Fear-avoidance behaviours are often seen with lower back pain so this is an episode many will resonant with and is packed with great content on how to move forward successfully. KEY TAKEAWAYS Fear-avoidance behaviour is avoiding a certain movement because it hurts Avoiding a movement because it hurts is part of the body’s protective mechanism It’s natural to think that resting and avoiding the movement will help with recovery but the opposite is true Long-term avoiding the movement can mean you become even more sensitive to the movement and this can result in increased pain You can become trapped in a vicious circle and this is especially common in lower back pain When a movement is avoided for an extended amount of time then the brain can become conditioned to create pain based on thoughts, fears and belief If you haven’t completed a specific movement in a very long time there can be a significant amount of anxiety around making that movement Breaking the cycle of fear-avoidance behaviour is very important as it can lead to anxiety, depression, a decreased quality of life and persistent pain Graded exposure therapy is about slowly exposing someone to something It involves gradually and repeatedly exposing the mind and body to painful stimuli to retrain the brain BEST MOMENTS ‘When a movement hurts, we avoid it and its totally normal’ ‘Understanding the movement is normal and that the spine is meant to bend and lift is vital’ ‘You need to understand what pain is and that the body can generate pain in response to a perceived threat’ THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. The Problem with MRIs for Back Pain
Rob and Dave talk about all things MRIs in this episode and share great information on the role and importance of MRIs as part of the medical diagnostic toolkit for back pain They also discuss why it’s the reporting and lack of presentation that can be the problem and how this information must be used in correlation with other symptoms for a working diagnosis. If you have ever wondered why, you weren’t offered an MRI, refused one when you asked or want to understand more about MRI’s and back pain then this is the episode for you KEY TAKEAWAYS An MRI is a fantastic diagnostic tool it’s the reporting or lack of reporting that can be the problem An MRI shows everything that is under the skin and most people will have something that can be blamed for the pain Standardising the regular use of MRI’s can potentially lead to attributing some normal age-related factors with pain and going down the wrong route It’s about the correlation with your other symptoms and looking at the overall picture of presentation It’s important to make sure that information is carefully shared so there is an understanding MRIs are an incredibly important part of the medical diagnostic team but it’s about being careful about how the information is disseminated Therapists know from the assessments they complete what the red flags are and what they are looking for A ‘just in case’ MRI can be confusing because it shows everything With an MRI you must take into account all the other diagnostic information A red flag raises the suspicion that there may be something else going on, but it doesn’t mean there actually is anything to be concerned about If you do self-refer for an MRI, then take the report to your professional BEST MOMENTS ‘It’s the presentation or lack of proper presentation to the patient that is the problem’ ‘We know pain is multifactorial’ ‘Therapists are really good at knowing what the red flags are’ ‘Words and presentation have to be chosen carefully’ THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. What Everyone Should Know About Back Pain
In the first episode of this mini-series, Rob and Dave begin the discussion about what everyone should know about back pain by looking at pain. Pain is real for anyone who is suffering with it but Dave and Rob explain why it doesn’t always mean damage and how the brain can both up-regulate and down-regulate the experience of pain. Listen in and learn how your experience of pain is impacted by your prior experience and attitude and how to make positive changes. KEY TAKEAWAYS People equate pain with damage but this is not necessarily the case The definition of pain is an unpleasant sensory or emotional experience in relation to actual or potential tissue damage When your brain perceives a potential threat the only way it can tell you is by creating pain Pain is totally controlled by the brain and your brain can up-regulate and down-regulate pain signals The brain takes into account many factors including previous experiences Your brain can override if it has something else it’s focusing on We all have the ability to reduce pain we just have to learn how to harness it There are many different nerves in the body and they are activated by different stimulus. All information from your nerves is sent to the brain where it is decoded Having pain doesn’t mean there is any damage The brain can up-regulate or down-regulate what you experience, based on your experience, outlook and attitude Pain can have many, many triggers but the pain is real and the experience of it is the same You can manage pain and there are lots of ways to do this. BEST MOMENTS ‘Your pain is real, if you are feeling pain it is real’ ‘You are able to influence how you react to and manage your pain’ ‘Pain is real for anybody who is suffering from it EPISODE RESOURCES Monty Lymans Book - The Painful Truth Link https://amzn.to/3j0lHhZ Sticks and stones book http://www.therapteuticalliance.com/store/p2/sticksandstones.html FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. How to Get the Right Therapist for You
Any treatment plan needs to be one that works and in this episode, Dave and Rob share a great overview of what to look for to get the best therapist for you as an individual. Because we are all completely unique and individual every good therapist will create a personalised treatment plan, designed to achieve the goals and outcomes you have discussed together. Listen in and hear about the key things to look for so that you get the therapist that is the right one for you KEY TAKEAWAYS Every treatment is different and can take different amounts of time and may have different responses because everyone is individual You should leave every session feeling better in your movement, pain and outlook Is your therapist installing a positive outlook about your pain and your outcome? Have an agreed goal with your therapist, an end goal that you are both working towards Any therapist should give you the time to ask questions and answers may change the treatment plan Whatever the treatment the therapist should be able to tell you the reasons why they are doing it The opportunity to speak and talk is vital A therapist must listen carefully to what you are saying and actively focus on what you, the patient, are saying Building rapport and a relationship between the patient and the therapist is vital There needs to be an active component to any care along with the passive care The active care is the things you can do yourself; the passive care is what is done to you. Your practitioner should be giving you the tools and educating you about your condition and treatment BEST MOMENTS ‘If they pass our tests chances are they are able to provide the best quality care that works for you’ ‘Optimism and outlook are as important as treating the function and the pain’ ‘We want to update and bring you into the treatment because we know that’s what works’ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ ABOUT THE HOSTS Dave Elliot Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Why Be Resilient When You Can Be ANTI-FRAGILE – A Patient Story
Anti-fragility is the next level in resilience and creating robustness, it’s about not only bouncing back but bouncing back better and stronger. Rob and Dave explain the concept of anti-fragility and why it can make the difference for back pain with graded exposure to stressors helping you to develop better adaptation to the unknown stressors we all face each day and brings growth on the other side of stress. This look at a philosophical concept is a great reminder about the importance of both the mental and physical aspects of recovery. KEY TAKEAWAYS Improving resilience is about improving your response to stress You need to develop more resilience and become more adaptable You get stronger when you stress your muscles during exercise If resilience is about your ability to resist stressors and being able to remain the same in response to stress Anti-fragility is the ability to get better in response to stress The daily stressors that we face are unknown so we need to get better at adapting to them Anti-fragility is about growing and getting better through adaptation to stressors It’s about growing on the other side of the stress If responses to stress make us stronger then the opposite is true with a lack of stressors meaning we get weaker Graded exposure to stressors is vital in becoming stronger Your adaptation doesn’t just have to be cellular it can be mental as well Graded exposure to pain through repeated movement can be very helpful The mental model of anti-fragile can be applied across all areas of life It’s about moving better and being able to bounce back better and stronger BEST MOMENTS ‘The back pain podcast is sneaking in some philosophy’ ‘The mental side is as important and sometimes more important than the physicality’ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ Antifragile: Things that Gain from Disorder - Nassim Nicholas Taleb ABOUT THE HOSTS Dave Elliot Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Exercise Vs. Back Pain; What Does it Do? with Adam Meakins
Physiotherapist Adam Meakins gets into the detail about what good exercise is and how the right exercise can help individuals to build strength In today’s society, humans move less but it’s really important to move more - that’s what humans are designed to do and Adam explains how exercise exposes you to effort, challenge and difficulty and means you are more robust and resilient to all challenges in this focused and informative look at why exercise really does make the difference KEY TAKEAWAYS Exercise is anything that is effortful and challenging and physical activity is something that is not effortful The world health organisation guidelines recommend a minimum of 300 minutes of exercise per week The sessions of exercise need to last for longer than ten minutes The recommendation is for at least two sessions of resistance-based exercise weekly covering all muscle groups in the body Resistance can be applied by an external force or the body Exercise makes things better by initially making things worse When we exercise we cause adverse reactions - blood pressure rises, stress increases hormones are released and micro-damage to muscle tissue is caused. Exercise triggers processes that are positive responses and means our bodies super compensate Exercise helps to reduce a multitude of pain even though we don’t fully understand how You can’t avoid pain but you can reduce your risk through exercise Exercise exposes you to effort, challenge and difficulty and this means you are more robust and resilient to all challenges Exercise helps people both physically and psychologically There is no such thing as a bad exercise only those that are poorly prescribed Attaching meaningful goals to exercise can be motivating Any movement is a dynamical system and all human movement is dynamical BEST MOMENTS ‘What exercise is can be defined by being effortful’ ‘Exercise helps to repair our DNA’ ‘Good exercise is something that makes you feel good’ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ The NAF Physio podcast The Sports Physio ABOUT THE HOSTS Dave Elliot Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. All About Axial Spondyloarthritis-AxSpa-Ankylosing
Zoe Clark, osteopath and educator with a special interest in Axial SpA talks about the condition and how although it affects around 1 in 200 people in the UK currently the average time frame from getting symptoms to diagnosis is eight and half years. As a professional treating patients Zoe is also someone with Axial SpA and in this episode offers great advice and information on this condition, a common cause of non -specific back pain and discusses both the symptoms that can be indicators and the possible reasons there is such a lack of awareness leading to the very extended diagnostic timeframe. As always with The Back Pain Podcast, there is great content that will help anyone increase their understanding KEY TAKEAWAYS Axial SpA is a type of inflammatory arthritis and is generally inflammation in the body One of the first symptoms can be back pain and it affects 1 in 200 people in the UK Axial SpA is the umbrella term and MRI scans now help identify the specifics With inflammatory back pain, the symptoms usually get worse when you are still and better when you move Axial SpA can affect different areas of the body including the skin and the digestive system Putting your shoes and socks on can be the point when you experience pain Blood tests combined with symptoms and X rays all help with diagnosis The average time, in the UK from getting symptoms to diagnosis, is eight and half years A combination of factors contribute to the extended diagnostic timeframe It’s a common condition in the general public but there is a lack of awareness with people putting their symptoms down to injury Most people take anti-inflammatory medication Exercise and movement are the best things you can do. Find something you love and do it regularly Lots of people find manual therapy really helpful Stopping smoking will make a huge difference Smoking increases the risk of joint changes and other health conditions and the effectiveness of medications Give yourself time to process and accept the diagnosis BEST MOMENTS ‘Most people manage with anti-inflammatory medication combined with exercise’ ‘If there is one thing you should do with Axial Spa it is to stop smoking ‘NASS is a great source of information and resources for anyone with Axial SpA’ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ NASS website ABOUT THE HOSTS Dave Elliot Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Biggest Back Pain Misconceptions - Rob's interview with the Sports Therapy Association
Rob is a guest on the ‘Sports Therapy Association podcast’ and discusses the five most common misconceptions connected with lower back pain. Rob discusses posture, manual therapy, MRI’s and whether pain always equals damage along with the language used in the consulting room and the importance of building the right narrative. He also shares great content and information about the range of tools available for recovery and the importance of building a relationship with the patient. This is a great listen and anyone listening will learn something new. KEY TAKEAWAYS There is a misconception around the role of chiropractors A chiropractor has a myriad of tools at their disposal Your identity is what you do on a regular basis Chiropractors have evolved in what they do You must never ignore the patient narrative Lower back pain is usually multi factual and is rarely done to a single structure Our job as therapists is to encourage the narrative that you are strong and stable It’s vital to build rapport with the patient and you need to have this skill as a therapist. The vast majority of lower back pain is non-specific this doesn’t mean we don’t know the cause more that we can’t identify the specific structure So many factors influence the brain’s role in pain Addressing back pain has to be a multi factual approach Posture -it’s not the position it’s the lack of movement that causes the problem The problem isn’t with MRI’s is the poor reporting to clients. An MRI is a snapshot of what’s happening at that moment and results should be shared correctly with the patient. Previous beliefs can impact hugely on the journey of recovery BEST MOMENTS ‘We wanted to get the voice of the patients out there’ ‘All the tools are very valuable’ ‘People who spend time thinking about posture are more likely to suffer from backpain’ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ ABOUT THE HOSTS Dave Elliot Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK.
