Core Bracing, Pelvic Tilts and Massage Guns – Ask Us Anything
In this ‘ask us anything’ episode hear about pelvic tilt, asymmetry, and the answers to whether massage guns help and if core bracing is beneficial Rob shares great content and clear explanations about what actions will make a difference and discusses the importance of movement in this informative episode KEY TAKEAWAYS Pelvic tilt is commonly blamed by people for their pain Asymmetry is perfectly normal in the human body and doesn’t correlate with increased pain The body adapts best to being outside of neutral comfort zones Our bodies are designed to bend, lift, stretch and twist moving beyond the neutral zone The back benefits from load and this can reduce pain Massage guns help you to undertake soft tissue work Soft tissue work can help reduce pain for some people changing the input from a painful area of the brain If you want to increase blood flow to an area moving is the most effective way to achieve this is by moving Often core bracing is used in the short term Bracing for the first few days is fine but you should be exploring movement Often patients with back pain think they have a weak core but in reality, patients with back pain have an overactive core Deep breathing and movement can help with relaxing the core BEST MOMENTS ‘Sometimes bracing your core can help you to initially achieve a movement with less pain’ ‘What people have been avoiding can be the key to unlocking recovery and improvement’ THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ RESOURCES FOR THIS EPISODE Adam Meakins Blog - Leg Length Differences https://www.thesports.physio/the-long-and-short-of-leg-length-differences/ Mehmet Gem - Twitter - LLD and Pelvic Tilts https://twitter.com/thehipphysio/status/1483178257319174145?s=21 Greg Lehman - Core Stability Episode https://podcasts.apple.com/gb/podcast/the-back-pain-podcast/id1511515312?i=1000500403667 ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. What Everyone Should Know About Back Pain - Part 3
Rob Beaven goes solo this week and shares more great tips about changes you can make that will have a cumulative positive impact on your pain He discusses why building resilience and becoming more anti-fragile really can make the difference and how making changes to your sleep environment and recognising the role stress plays in your pain can change your journey to recovery KEY TAKEAWAYS A lack of sleep can have a negative influence on your pain and the quality of your sleep influences your body’s ability to repair Remove any electronic devices and make your bedroom your sanctuary Melatonin is responsible for controlling your sleep weight cycle and artificial light can cause disruption to this hormone Excess stress can be a trigger for pain Stressing or worrying about an injury can cause patients to experience pain There are many ways to relieve stress including walking, meditation, and yoga The fear of doing a certain movement because it’s painful can lead to fear avoidance and worry which can make the pain worse Avoiding a certain movement for a short while can help but graded return to the movement is important in recovery Pain is multifactorial there are many factors that lead to an episode of pain BEST MOMENT ‘Poor sleep equals poor repair’ ‘We sleep far better in colder darker rooms’ ‘It’s incredibly common for our patients to experience pain around a time of extreme stress’ THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain PodcastThe Back Pain Podcast websiteThe Back Pain Podcast recommended products affiliate linkOur Rode Mixer https://amzn.to/3waU8bxOur Microphones https://amzn.to/3rzSZ9Second Microphone https://amzn.to/2ObKMeAXLR Cable https://amzn.to/3rBL8ZBStudio Headphones https://amzn.to/3u082LELaptop https://amzn.to/3dhfafTOur webcam https://amzn.to/31uUefQ RESOURCES FOR THIS EPISODE Greg Lehman Articlehttp://www.greglehman.ca/blog/2018/5/1/do-our-patients-need-fixing ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Sticks and Stones, How Language and Words Can Influence Your Pain
Communication isn’t just words, it affects the way we think and feel and in this, not to be missed, episode Jarod Hall, physical therapist, and co-author of ‘Making Sense of Pain’ talks with Dave and Rob about the importance of communication and the way it impacts on us all differently according to our personal history, experiences and beliefs He shares great content for clinicians about the ways to explain injuries and symptoms effectively and empathetically, along with great advice for patients about understanding more about their pain and personal journey of recovery. KEY TAKEAWAYS Human beings are a social species and we communicate both verbally and in other ways When we use language we convey a lot of information Language and the way we speak to people has a huge impact on the way they feel Your physiology can change according to the words being used towards you You should communicate in a way that is helpful, not harmful Your thoughts affect your physiology and what people say to you affects the way your body functions and in turn the way you feel Exposure to painful stimuli and building over time with graded exposure can lessen the pain Clinicians need varied anecdotes to help different patients understand the processes of recovery Everyone is different and every interaction between clinician and patient means each person is bringing their experience and history and expectations to that individual situation It’s about working out the best way to interact with each other in the intersubjective space to achieve the goals Clinicians need to be able to assess when people are ready to hear what they have to say It doesn’t matter how great your stories are if you don’t have an open and respectful dialogue You need to ask permission to share your explanations as a clinician BEST MOMENTS ‘When we communicate with someone in pain we need to be even more aware of the language we use’ ‘As we age we all get wrinkles on the inside’ ‘His book looks at many of the analogies and stories surrounding pain and how the language used impacts’ THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ RESOURCES FOR THIS EPISODE Jarod's Book - Making Sense of Pain (Sticks and Stones) https://amzn.to/3JKSjc0 Jarod Instagram https://www.instagram.com/drjarodhalldpt/?hl=en Modern Pain Care https://modernpaincare.com/?doing_wp_cron=1641367942.7520160675048828125000 ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Everyone’s an Individual
In this special New Year’s episode, Rob and Dave say a huge thank you and reflect on some of their favourite bits from the last 12 months discussing back pain with the world’s foremost experts. It’s been a successful year for the podcast because of the people who have reached out and asked the questions that have formed the basis for the episodes so if there is something you want to hear about back pain get in touch and help shape the episodes for the New Year KEY TAKEAWAYS Everyone is an individual it’s about finding what works for you There are a range of styles of treatment Humans and their pain is not simple, pain is multifactorial and what you experience is based on many factors Individuality in pain should not be overlooked, you need to find the right combination for you Your body changes and something that doesn’t work at the moment may work in the future Therapists need to help patients to have self-management skills You should have self-management tools that mean you can help yourself Ask your practitioner what tools you need to self-manage when your back pain wakes you at 3 am in the morning BEST MOMENTS ‘Humans are messy, not linear or cookie-cutter what works for someone else may not work for you’ ‘If something doesn’t work for you it doesn’t mean your body is broken’ ‘You need the tools to be able to help yourself to get better’ THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. What's the Best Sleep Position to Avoid Back Pain?
Rob and Dave take a look at the question of the best sleep position to avoid back pain and explain what everyone can do to get the best quality of sleep Ultimately good quality sleep plays an important role in any recovery so this should be the focus, getting the sleep is always more important than how you sleep, listen in and hear more KEY TAKEAWAYS The best position to sleep in is the one that doesn’t cause you pain There is not a best position to sleep in to avoid back pain long term There is no evidence that adopting a specific position will help to improve back pain A big factor in any recovery is the quality of sleep You don’t often wake up in the same position as when you went to sleep because you move during the night If a position causes you discomfort then changing it for a short period can help A big factor in recovery is good quality sleep It’s all dependant on you and what’s comfortable for you as an individual Clinicians should be helping people to self-manage Your preferences may change over time in the same way your anatomy will change over time If you are thinking about a new mattress try out firm and soft toppers to find what works for you BEST MOMENTS ‘Whatever makes you comfortable is the position for you’ ‘Sleep is more important than how you sleep’ ‘Find what’s comfortable for you whether it's firm, soft, hay, straw or rocks!’ THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. What Everyone Should Know About Back Pain - Part 2
Rob and Dave continue their discussion around what they talk most about with patients on a daily basis. In this episode, they bust the myths and misconceptions about whether your back is fragile and weak and look at the narrative around ‘putting your back out’ They share great information and content in a way that is easy to understand and explain how they work with their clients towards successful recovery building strength, resilience and adaptability KEY TAKEAWAYS Your back isn’t fragile, the back is one of the strongest parts of the body Pain back can be some of the worst pain experienced so people become fearful of hurting their backs Our backs put up with a huge amount of stress on a daily basis The type of stress we put our backs through is a good thing The stress we place on our bodies is how we grow and get stronger, our muscles and joints require stress and work to grow Your back will become weak if you don’t use it at all because you won’t be exposing it to anything new Exposure to load is a really important part of recovery Breaking down a movement that is a trigger for back pain into small parts is part of effective recovery You gradually expose the spine and body combining the movements and adding load and as a result, get stronger Your back and body are inherently strong and adaptable if you put gradual and increasing stress on them False beliefs and misconceptions can mean that people become afraid of doing certain movements because they think it will put their back out Clinicians and therapists should be helping clients to improve their resilience and helping them to achieve improvement BEST MOMENTS ‘We are strong, resilient and adaptable and put up with stresses very well on a day to day basis’ ‘The stress and work we go through is what helps our backs to recover from injury’ ‘It’s become a common narrative in our culture to see the back as being weak’ THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Join your host Rob and Laura Rathbone, a pain specialist and clinician who in this episode talks about what it’s like to have chronic pain and how Acceptance and Commitment therapy can play an important role in being able to move forward. There isn’t universal biology for pain, it’s a nuanced experience created in the individual. Pain has psychological experiences within it along with biological processes and physical experiences. Acceptance and Commitment therapy offers an approach that is focused on acknowledging the struggle and looking at changing a dimension of the experience. Listen in and learn more to find out if it may be the approach you need to help with your pain KEY TAKEAWAYS You recognise the expertise of the person experiencing pain, they know their pain It’s about looking at the complete landscape of evidence to see if there is anything that can be used to help them with their pain The first conversation is a discovery session and beginning the process of working collaboratively Acceptance and Commitment therapy is developed as a third way of CBT and is a compassion-focused process-based therapy ACT is about acknowledging the struggle and looking at changing a dimension of the experience Pain has psychological experiences within it along with biological processes and physical experiences ACT has a structure and uses a person’s ability to self-observe their present moment, recognise where there are obstacles and take action We are using ACT to see if there is a way to create room for change It’s focusing on the principle of rehabilitation and moving flexibly with what life offers and staying connected to what’s important ACT is inviting people on a journey of exploration setting their own boundaries as to how they want to live well with the experience of pain and understanding that the pain may remain It’s about being willing to have the feeling to get to the stuff you want ACT is about trying to stay true to someone’s value-based action and stay in that moment of turning towards it An ACT informed approach to physical therapy is using the principle of rehabilitation in the moment with the added skill of managing the emotional distress BEST MOMENTS ‘They know their pain more than I can ever know their pain’ ‘I’m an MSK trained therapist looking to use psychological perspectives and skillsets within my therapy’ ‘Pain is changeable it’s just by how much and how it is done’ THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ RESOURCES FROM THIS EPISODE Laura Rathbone website Pain Geeks ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Spinal Stenosis - What is it and How to Manage it - Adam Dobson
Dave and Rob are joined again by specialist back pain physiotherapist, Adam Dobson and are discussing spinal stenosis They talk about what lumber spinal stenosis is and why for some it creates no problems and for others, it will mean treatment to alleviate symptoms and improve quality of life Adam shares great information and advice on what to do if it does cause you problems and explains how exercise therapy and surgical decompression are used based on clinical assessment. This is a must-listen with great answers for people with lumbar spinal stenosis KEY TAKEAWAYS Lumbar spinal stenosis refers to the lower back. The symptoms are defined as being positionally influenced or exertionally influenced Three-quarters of people over the age of forty are expected to have some degree of spinal space narrowing 11% of the population present with these problems but it’s not a sign of the body wearing out Some masquerading problems may not be related to the back at all Walking even with a degree of leg pain is not harmful if it's tolerable Bending the back for therapeutic effect should be encouraged There can be forms of general exercise, sleep and weight management that can help For most people, lumbar spinal stenosis is a long term problem The focus should be on quality of life function and exercise is a good first-line treatment It’s not about trying to change the tunnel narrowing it's about influencing the biology, how things feel and inflammation around the nerve roots. If the pain is acceptable then it’s fine to continue and working with a clinician can help to establish your personal baseline Surgery is often reserved for those with high degrees of disability and those who have already been through physiotherapy, it's rarely essential so preference plays an important role BEST MOMENTS ‘There will be softer signs for spinal stenosis’ ‘Narrowing doesn’t mean you will have problems’ ‘I’m a big advocate of getting the back bending’ ‘We are trying to make things feel better and improve the condition’ THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ RESOURCES FROM THIS EPISODE Spinal Stenosis Booklet https://www.southtees.nhs.uk/content/uploads/Lumbar-Spinal-Stenosis-Booklet.pdf South Tees NHS patient information booklets https://www.southtees.nhs.uk/services/back-pain-triage-and-treat/patient-information-educational-resources/ Adams Twitter https://twitter.com/adamdobson123 ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Why are Opiates no Longer Recommended for Chronic Back Pain? - Diarmuid Denneny
Rob talks with Diarmuid Denneny a lead physiotherapist, Chair of the Physiotherapist Pain Association, and member of the guideline development committee for the NICE chronic pain guidelines in an episode that shares great informative content about the topic that is currently very high profile – the recommendations around prescribing opioids for back pain. Diarmuid explains what the recommendations are and discusses with Rob the vital elements of a collaborative approach and informed decisions along with alternative strategies in the effective treatment of pain where it’s all about starting small and building KEY TAKEAWAYS The NICE guidelines are evidence-based recommendations for health and care The advice is to not initiate a prescription for opioid medication for chronic pain Opioids have a place in the treatment of shorter-term pain and for end of life care It’s about making sure that people who are taking opioids are fully aware of the potential side effects If someone is taking opioids at a low dose and is seeing benefits it’s about a shared care plan and the individual being fully aware of the risks It’s about informed choice and shared decision making with the prescribing professional There are many factors to consider and the conversation with the clinician at the beginning is key in a personalised approach It’s important to consider alternative strategies It’s always a collaborative approach between the patient and the prescribing clinician A range of strategies should be considered around the pain being experienced and what the individual wants to achieve There are factors that influence pain that are beyond your control such as the environment Pain is complex and there are elements we are aware of and many that we are less aware of and have less control over It’s about medication helping with the management and augmentation of pain BEST MOMENTS ‘People expect that because they are called painkillers they will stop the pain but for most it’s a reduction in pain’ It’s not about stopping opioids for everyone ‘It’s starting small and building gradually THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ RESOURCES FROM THIS EPISODE Opioids Aware Live Well With Pain Footsteps Festival 2021 Facebook ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK.
