#101 - The Knowlege Exchange Podcast - Explaining Misconceptions & What is High Value Care. Rob's Guest Appearance

Rob is a guest on the Knowledge Exchange podcast talking about misconnections and what good quality care looks like He discusses the power of the words and language used by practitioners and explains why he is an advocate for clear and simple communication with patients. Practitioners need to understand where patients are coming from and acknowledge their beliefs working with them to support them in achieving their goals. Being a high-level communicator and sharing strengths is crucial for success for any practitioner who is working with patients   KEY TAKEAWAYS MRIs are a valuable tool the challenges are in the reporting An MRI shows everything, they need to be explained carefully and used in conjunction with your medical history and consultation Posture is ingrained in our culture but can be misunderstood The problem isn’t the position you sit in it’s the movement you make Postural devices don’t help with posture Communication is key and we always want to communicate with patients in a clear and easily understood way A summary at the end of consultation can really help to make sure the patient leaves with the right information and understanding Acknowledge the patient's beliefs and work with them to change the narrative Building trust with the patient and understanding their goals and beliefs is crucial As healthcare professionals, we have a responsibility to make sure we are sharing correct and factual information We have the opportunity to educate and inform people, we are primarily patient-facing and want to put out clear simple information for people We want patients to know what to expect when they meet with their healthcare practitioner Having groups means we can understand more about what people are going through and is the opportunity to share and signpost potential support Passive care is when something is done to the patient and active care is something the patient does for themselves    BEST MOMENTS ‘MRIs don’t cause back pain, it’s about the reporting ‘For patients, it’s their story’ ‘You have to meet patients where they are at and begin from there’   THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/   VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ    RESOURCES FOR THIS EPISODE The Knowledge Exchange Podcast    ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Pain and Motivation in the Journey to Recovery

Dave explains why it’s important that your recovery is not just measured by an improvement in your pain levels but also by progress in the function and movement you want to achieve from being able to run a marathon through to being able to pick up your child Pain maybe the tipping point that led to you seeking intervention but your ‘why’ the motivation and goals you have are crucial in your journey to recovery and the function and movement that matters to you   KEY TAKEAWAYS Understanding the motivation of the patient and their goals for health and movement is vital Everyone wants to get out of their pain Pain is a major motivator – no one wants to be in pain and its often the first thing that’s mentioned What function and movement do you want to have when you are pain-free? Pain is easily measured and quantified as you progress but shouldn’t be the only measure Pain and recovery are not linear so if pain is the only measure seeing  progress clearly can be challenging Pain may remain in the background and is very subjective. It’s felt differently depending on many variables Let your clinician know your goals and the activities you want to return to Function goals can be hugely positive in the journey of recovery Additional data about what you want to do means your rehabilitation plan can be very specific   BEST MOMENTS ‘A real and personal goal can be the motivator you need’ ‘Pain can be the tipping point ‘   THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/   VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ    ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. What Type of Headache Do I Have – Elizabeth Huzzey on the Most Common Headache Types

Elizabeth Huzzey who has over 30 years of clinical experience specialises in headache disorders and in this episode discusses the development and increasing knowledge about different types of headaches that can be experienced Understanding your headache is vital so that you can choose how to manage your headaches and seek help from the right professional Elizabeth shares informative content on the various types of headaches and discusses some of the ways headaches can be managed effectively, listen in, and find out more    KEY TAKEAWAYS 99% of the time headaches are not something to worry about but if you have a sudden onset ,severe headache you should seek immediate help There are three hundred different types of headaches and they are divided into two types – primary and secondary Primary headaches are divided into 3 categories, tension, migraines, and TAC Most of us will have a tension-type headache at some time A migraine is a neurological event that you are generally genetically predisposed to A migraine with aura begins with a small electrical wave going through the outer part of the brain. It’s a sign your migraine is about to happen The typical migraine headache is one-sided, unilateral, and generally over the eye and side of the head When experiencing a migraine headache, it is difficult to function Activation of the nervous system causes an expression of a neuropeptide and the protein creates a sensitivity in part of the brain Triptans stop the pain of the headache because they stop the dilation of blood vessels if taken in time CGRP blocks the neuropeptide and can stop the headache from beginning TAC headaches are always on one side and the extreme pain can result in people feeling very agitated The hypothalamus is involved in cluster headaches so the headaches can often happen seasonally at the same time of the day   BEST MOMENTS ‘Visual auras are very common’ ‘Most people will need to find a quiet dark room and wait for the storm to pass’ ‘When you have experienced an aura repeatedly it becomes a familiar sign that a migraine is beginning’   THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/   VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ    RESOURCES FROM THIS EPISODE Watford Osteopaths   ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Is It OK to Round My Back When Deadlifting – The Risks of Getting out of Neutral with Dr Louis Howe

In this episode, Dr Louis Howe researcher and strength and conditioning coach discusses lumbar spine flexion and getting out of neutral He explains the current research and how in real life spinal flexion can’t be stopped but can be managed with the right strategies to avoid end range motion This is a fascinating episode for both professional practitioners and individuals who want to understand more about spinal flexion and what getting out of neutral really means   KEY TAKEAWAYS Neutral alignment is seen as being optimal in sharing the load The research indicates that the spine flexes away from the neutral posture Whilst flexion can’t be stopped there are strategies that can be used to avoid end range motion and lower or increase those values When you perform lifting you are flexing and moving out of neutral We see more flexion occurring at the lower lumbar spine than at the upper lumbar spine When you are lifting the spine is both flexing and being exposed to compressive load Flexion has the potential to result in pathologies of the spine All of the tissues in our bodies can adapt to loading There is potential for managing the position of the spine and affecting injury risk Avoiding end range position and flexion can be important in training Coaching can encourage kinesthetics awareness to help avoid end range positions Programming strategies and the way overload is created in exercise is valuable as an injury risk management tool Years of strength training will bring architectural changes and spinal flexion can put muscles in a better position for lifting   BEST MOMENTS ‘The concept of neutral is attractive to practitioners because it puts the spine in a position where the load is spread’ ‘It’s a variable that we can manipulate’ ‘When they perform the tasks they do it in such a way that they are able to cope with the loads they are exposed to’   THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/   VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ    RESOURCES FROM THIS EPISODE Getting Out of Neutral: the risks and rewards of lumbar spine flexion during lifting exercises - Louis Howe with Greg Lehman   ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. The SLEEP Episode – How to Get Great Sleep and Overcome Insomnia

Dave talks with Tracey ‘The Sleep Coach’ about what leads to poor sleep and insomnia along with how you can change things to achieve good sleep A good night’s sleep is the foundation for feeling good and being able to do what you want to do during the day so good sleep is always important Tracey shares practical tips on what you can do to optimise your sleep so that the sleep you get during the night is ‘a good night’s sleep’ making you feel good and able to do what you need and want to do during the day   KEY TAKEAWAYS Sleep is very individual; some people require more sleep than others Optimise your sleep by creating the right environment There is no need to try to ‘catch up’ on missed sleep It’s important to keep doing the good things in life to take your attention away from any sleep issues A relaxed approach to sleep is the best way to achieve it An uncluttered room, a comfortable bed, a cooler environment, and a good wind-down routine are the starting points for good sleep Good sleepers can’t tell you how they sleep they just go to bed and sleep well Tracking sleep patterns only gives you information to hang anxiety on so stop clock watching your sleep and ditch the wearables There is not one position that is preferable for sleeping, it’s very individual and depends on several factors Sleep is an emotional process as well as a physical one and it’s not always linear Sometimes we lose our connection or get in the way of it but you can reconnect with your natural ability to sleep it’s something we never lose CBTi gives you a set of skills mental, emotional, and behavioural that you can use for the rest of your life   BEST MOMENTS ‘When you stop worrying about sleep it lowers your arousal level and you will experience better sleep’ ‘There isn’t a return for trying to get more sleep than you need‘ ‘Accepting that you may not sleep well takes the anxiety away’   THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/   VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ    RESOURCES FOR THIS EPISODE Tracey The Sleep Coach Tracy The Sleep Coach Instagram Sound Sleep Strategies Group Facebook   ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Understanding my MRI results - DDD, Disc Bulges, Annular Tears and more?

Rob and Dave are sharing invaluable content today as they explain some common imaging findings and how they would discuss them with their own patients Having someone explain your scan to you is really important because, as Rob and Dave explain any diagnosis is made from a combining of  the information from your medical history, your physical examination alongside any imaging There are normal age-related changes that happen to everyone and the skill of the clinician is working out which changes are normal and what may be contributing to or causing your pain, listen in and find out more   KEY TAKEAWAYS The diagnosis your clinician makes is based on a combination of your medical history,  a physical examination and this is then combined with your imaging An MRI, X-ray or CT scan can’t see your pain, they are a snapshot of how you look at that point in time Back pain is very common without there being any changes on a scan Interventions can be about modulating and reducing the pain If your scan comes back as normal it doesn’t mean you don’t have pain It’s a win and a great starting block if your scan comes back as normal Normal age-related changes (NARC) happen to everyone and are expected over time If you’ve been told you have a degenerative disc disorder it doesn’t mean your pain will get worse Focusing on incidental findings can mean focusing on the wrong areas, and not achieving the proper outcome Changes happen to everyone with or without pain. It’s working out if the changes are relevant or irrelevant Disc bulges can occur without pain and often it’s the location that’s more important Disc bulges happen very slowly over time and the disc doesn’t have to go back in for the pain to go away A scan is just a snapshot in time to be used alongside other information to discover the likely cause of your pain   BEST MOMENTS ‘The job of a good clinician is to describe the changes and explain them to you’ ‘A scan is another tool in the arsenal to finally land on the conclusion’ ‘There are often words in reports that can make you think the worst when not understood in the right context’   THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/   VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ    ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Is Bending Bad for Your Back?

Should I stop bending over? We are taught early on about how bending or lifting must be done in a particular way to protect our backs but is there a right way to do it and is bending a trigger for back pain? In this episode, Rob shares great content and the answers to the questions people are always asking about bending and its connection to stress and pain. He also explains how to build and prepare your body to help it adapt, listen in, and find out more   KEY TAKEAWAYS People often blame bending as a trigger for their back pain Your back can be hurt in the same way as any other part of your body It can be easy to become fearful about hurting your back There are always many factors to consider if you have back pain There is not a correct way to lift, although there may be an optimum Apply the rule of twos - Are you lifting too much too often after lifting too little for too long Bodies are amazing at adapting to stresses if they are prepared Stress is how the body grows and adapts It's all about building your body to cope with increased load   BEST MOMENTS ‘Our bodies really don’t like sudden change’ ‘If you never bend from your back you can’t expect to be great at bending from your back, you have to prepare’   THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/   VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ    ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. You Are Not Your MRI

Dave and Rob discuss a topic that reoccurs often in their Facebook community – MRI scans when to have them when they are not needed and using the right language to discuss them This episode is a great reminder about the importance of explanation and the right language around MRI’s and how they are an amazing tool but only one piece of the puzzle when it comes to the diagnosis of back pain. Listen in and hear great informative information   KEY TAKEAWAYS Patients can often expect that an MRI is going to show them where their pain is An MRI shows everything that is happening under your skin A clear and skilful explanation of an MRI is vital Language is really important when talking to patients about their pathway to recovery Changes on your spine seen on an MRI do not corollate with pain and your spine deteriorating over time There are a thousand different reasons why back pain can flare up An MRI is looked at along with your medical history, pain, examination, and all other information An MRI is just another piece of the puzzle It’s a very good tool but not something that needs to be used as the first step with back pain unless there are red flags   BEST MOMENTS ‘An MRI works really well when used with the right patient at the right time’ ‘MRIs are seriously cool and some of the best diagnostic imaging we have’   THE BACK PAIN PODCAST PROVIDER MAP - FINDING SOMEONE TO HELP YOU WITH YOUR BACK PAIN https://thebackpainpodcast.com/index.php/members-map/   VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast website The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ    ABOUT THE HOSTS Dave Elliott Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK.
