Episode notes
Diuretic Nephrotoxicity
- Diuretics themselves aren't nephrotoxic, it's often misuse causing hypovolemia that leads to kidney problems.
- Giving too much diuretic can cause problems, but it's the resulting hypovolemia, not the drug itself, that's harmful.
Furosemide Dosing
- Empirically titrate furosemide: if no urine in an hour, double the dose, and repeat as necessary to find an effective dose.
- Furosemide's bioavailability varies greatly, so precise PO-to-IV conversion is less important than finding a working dose.
Albumin and Lasix
- While albumin-bound diuretics must be transported to the tubule, giving albumin to improve furosemide delivery isn't definitively proven beneficial.
- The idea of giving albumin with Lasix is interesting, but more evidence is needed to support it in practice.
