Smokers can have heart disease with nothing else, in excess of small LDL particles. The science was clear even 20-some years ago. Grains and sugars, period. So I asked these people thousands of them to take grains and sugars out of the diet. They'd come back, whatever, three, six months later, and they'd say, I did it.
