Efficiency with small processes is not a significant system change; altering the system to prevent issues like filtering out red beads more effectively represents a meaningful system change. Leaders, particularly senior ones, play a crucial role in shaping and being accountable for the system. Blaming is not productive, but acknowledging that management may be constrained by larger systems like financial expectations is essential. Effective leadership involves accountability and guiding the system towards improvement; those unwilling to be held accountable should not lead as true leaders embrace accountability.
