LDL as a Causal Factor in Heart Disease

Efforts to reduce heart disease by raising HDL cholesterol have failed, while treatments aimed at lowering LDL have been successful. High LDL, particularly small LDL, is a pathologic agent that should be therapeutically lowered. In contrast, the role of HDL is more complex, being important for cholesterol removal but not a guaranteed risk reducer for heart disease. Genetic associations show that genes linked to high LDL are also connected to heart disease risk, highlighting LDL as the causal agent, whereas genes related to HDL variations are not linked to heart disease risk, distinguishing the roles of LDL and HDL in therapeutic interventions.
