Notärzte sind eine begrenzte Ressource hier und wenn ich trotz 5min Transport 30min aufwende, um den Patienten an die NIV zu adaptieren, ist das nicht sinnvoll.Folgen unvermeidlicher Maßnahmen antizipierenWenn ein Patient intubationspflichtig ist, aber noch kurze Zeit grenzkompensiert, der einzige Ausweg aus der Wohnung aber ein 40x80cm-Kellerfenster ist, dann verschiebe ich die Intubation auf draußen, weil das Gerät nur schwerlich zusammen mit dem Patienten auf dem Tragetuch durchs Fenster passt. Man könnte die Regel auch nennen: was muss muss, aber alles hat seine Zeit.frühzeitig an den Transport denken Passen wir durchs Treppenhaus? Wenn nein, was dann?„Jeder Patient erhält von mir 30 Sekunden Aufmerksamkeit.“ Jeder Patient erhält daher eine Ersteinschätzung nach ABCDEWenn Du glaubst, ein Patient könnte von NIV profitieren, dann fang JETZT damit an (außer Dir fällt auch direkt ein sehr guter Grund ein, es nicht zu tun). Es ist ja nachgewiesen, dass NIV umso besser funktioniert, um so früher man es beginnt. Und wenn der Patient darunter auch nicht entscheidend besser wird, hat man wenigstens die bestmögliche Präoxygenierungsstrategie gewählt.Hör auf zu glauben, dass in der Klinik alles besser funktioniert Dem Patienten ist es egal, wer die notwendige Maßnahme durchführt und wo sie durchgeführt wird. Wenn die Maßnahme indiziert ist, ist sie in der Wohnung oder im RTW indiziert oder im Schockraum. Es gibt keine Rechtfertigung ein wichtiges Medikament oder eine Atemwegsicherung an die nächste Instanz zu turfen, weil man „ja eh in drei Minuten da ist“ (meistens sind es dann ja doch nicht nur drei Minuten).Wenn man etwas „mal schnell“ machen will, wird es nicht funktionieren. Wenn man alles beim ersten Mal mit der gebotenen Sorgfalt und vernünftiger Vorbereitung macht, wird es am Ende am schnellsten funktionierenDie Wirkung von vielen i.v. Medikamenten ist entscheidend von der Injektionsgeschwindigkeit abhängig. Die unerwünschten Nebenwirkungen von Analgetika, Hypnotika und Vomex sind viel fieser, wenn man sie volley reindrückt. Wenn man aber will, dass Metoprolol eine Tachyarrhythmie terminiert, muss man genügend davon schnell injizieren (von Adenosin ganz zu schweigen).Wenn es Notfallsituationen gibt, die Dir Angst machen, beschäftige dich gerade mit diesen Skills vorab. Bilde dich fort und mach Dir mentale Modelle, damit Du in der stressigen Situation wenigstens für Dich weißt, was Du machen willst. Es wird nicht alles gut! Notfallmedizin impliziert zwingend und immer, rasch Entscheidungen zu treffen mit unvollständigen Informationen. Wenn Du als Notarzt auf dem Auto sitzt und noch nie intubiert hast, kein Kochrezept für eine Notfallnarkose hast, Dir Kindernotfälle Angst machen und Du nicht weißt, wie man eine Minithorakotomie machst, bist Du da falsch. Geh in den OP hospitieren, nutze FOAM, besuche einen PALS- und einen PHTLS-Kurs. Sonst bist Du kein Gewinn für den Patienten und den Rest vom Team. Wer das nicht kann, kann nicht in verantwortlicher Position in der Notfallmedizin arbeiten (weder als Notfallsanitäter, noch als RA, noch als Arzt).
