All About Spinal Stenosis
Rob and Dave talk Mr Alexander Montgomery, Consultant Orthopaedic Surgeon about Spinal Stenosis, a common condition responsible for much back, neck and arm pain. They discuss what Spinal Stenosis is the symptoms that are experienced and hoe you can have it with no symptoms at all along with the various treatments and surgical options available. Understanding what spinal stenosis is and what the best treatments are will ensure you know what to look for and how to seek the most effective treatment. KEY TAKEAWAYS Spinal stenosis is the narrowing of the spinal canal where there are nerves. You can go for an extended amount of time with some compression and no symptoms and then an incident, such as a fall, can be the tipping point for more severe inflammation The most common areas are the lower lumber and half way up your neck The narrowing of the spine canal and degeneration is connected with aging. It vital to build strength and mobility as you get older Spinal stenosis can cause weakness and you may get Corda Equina symptoms. Symptoms of numbness around the groin area or loss of control need immediate treatment to relieve the pressure. Go to a hospital where there are spinal surgeons to avoid time wasted being transferred from a smaller hospital. For mild to moderate symptoms movement rehabilitation that offloads the pressure from the specific points will work. A steroid injection is an option but its effectiveness is dependent on the position of the narrowing. The size of the compression doesn’t necessarily match the level of pain. Most people don’t require surgery other treatments are successful. Any nerve compression relief usually has positive outcomes Surgery is very successful at relieving pain BEST MOMENTS ‘Any nerve compression relief usually has positive outcomes’ ‘The steroid injection is there to break the cycle to enable movement rehabilitation to take place’ ‘When you have pain, you make adaptations to cope with it’ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast recommended products affiliate link Mr Alexander Montgomary link https://www.schoen-clinic.co.uk/alex-montgomery ABOUT THE HOSTS Dave Elliot Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. What is Cauda Equina Syndrome? - Rob Tyer
Cauda Equina is a condition many have heard of or been warned about it but what is it and what are the symptoms you need to look out for? In this informative episode, Rob Tyer physiotherapist talks about everything cauda equina and explains what it is, the symptoms to be aware of and why if you have cauda equina treatment is time sensitive. He also shares the types of questions you will be asked and how the answers will help professionals to decide on the right treatment pathway. KEY TAKEAWAYS It’s a collection of syndromes in and around the function of the bowel, the bladder, skin between the legs, sexual function and pain around the back. You can have symptoms of cauda equina or radiographic cauda equina, in order to have cauda equine syndrome you have to have a combination of the two The symptoms are very varied but can be any changes in the function to the bladder usually in conjunction with changes in bowel movement. Numbness between the legs can also be a symptom along with sexual dysfunction. If you have symptoms you need to seek emergency care. You will need to provide a history and there will be an examination and possibly a scan. If you have cauda equine syndrome surgery is usually offered because of the time sensitivity. There is no treatment that can be done non surgically to take the pressure off of the nerve. Surgery is the only way to relieve the pressure. BEST MOMENTS ‘Back pain and a change in bladder function doesn’t mean you have cauda equina’ ‘Try not to be embarrassed about the symptoms’ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast recommended products affiliate link https://clairepatella.com

Should I See A Chiropractor, Physiotherapist Or An Osteopath? - with Mike James
In this week's show, Dave and Rob welcome physiotherapist, Mike James - AKA The Endurance Physio - to the show to discuss the enduring topic of what the differences are between a chiropractor, a physiotherapist, and an osteopath, and how to decide upon which might be the best for you? KEY TAKEAWAYS All therapists and practitioners in the MSK realm offer something unique in terms of treatment, and so all should be considered for their own contribution. When looking for a therapist, we must search for the traits of a good one. These traits usually come down to how they communicate with you. A good therapist will always try to establish a good relationship with the client. The patient must remain at the core of every treatment. Good therapists will seek to collaborate with the patient in order to facilitate the path to the destination required. Research is everything. Look for good clinics, but also for good therapists within those clinics. Do not be afraid to raise questions. BEST MOMENTS 'Ultimately they all offer something to patients in the MSK realm' 'People have taken many journeys and paths to get to the same place' 'The patient must be at the centre of the process' 'It doesn't matter about the therapist's background - it's the person and the way they work' VALUABLE RESOURCES Mike James Facebook - https://www.facebook.com/TheendurancePT The Back Pain Podcast The Back Pain Podcast recommended products affiliate link ABOUT THE HOSTS Dave Elliot Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Why Do My Knees Hurt - Claire Robertson
People with back pain may also have other problems and in this episode hear all about knee pain, what the causes are and what you can do about it with expert Claire Robertson. Claire specialises in Patellofemoral pain syndrome and shares great information about what is actually happening in your knee and some great analogies that mean it’s easy to understand how the causes can be different for different people. It’s usually not about a specific event or injury but about a gradual change over time. Claire also explains how to be proactive in choosing the right exercise and movement so that you can build the strength that will make the difference. KEY TAKEAWAYS Patellofemoral pain syndrome is an umbrella term used about the pain caused when the knee cap rests on the bottom of the femur. It can happen at all different ages for a range of different reasons It can be a vague pain that is aggravated by walking upstairs or an incline. Often it’s about highpoints of pressure rather than damage. It’s about looking at changes that may have affected the load and movement pattern of the knee. Calf tightness can a contributing factor and taking the time to stretch out muscles before walking can make a difference and building up the strength in the quadriceps can help. Treatment is very multifactual depending on the drivers. It’s important to get an accurate assessment and use this as a basis for treatment. If injections are used it creates a window of opportunity to build strength for the long term. It’s about making sure you are doing the right exercise and movement in the right way. Knees are strong and designed to facilitate movement but if you have any pain you should get it checked. BEST MOMENTS ‘A change in lifestyle can be a factor’ ‘The cause can be different for different people’ ‘Normal knees crack and crunch during movement’ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast recommended products affiliate link https://clairepatella.com

Call in The Chiropractors
In this special New Year’s edition of the podcast, Rob and Dave are joined by Tom white from Salford city chiropractic and Ricky Davis from Summit well-being in London. They discuss some of the cases in clinic this week sharing great insights into their initial consultation process and how this lays the foundation for collaborative work between the therapist and patient on the journey to recovery. It’s always about the formulation of a tailored approach that’s right for each patient that supports their physical and mental wellbeing as they make incremental steps towards their final goal. KEY TAKEAWAYS When someone has pain and stops doing things then the body can get used to not moving You can use the examination to help patients to see what they can do. Even if you can’t do your chosen activity now you can move towards it. It’s all about the small wins and incremental progress towards your final goal. The definition of health and well-being is the physical, mental and social wellbeing of an individual. Even when you are in pain it’s about finding what movement you can do in the right way whilst the pain and injury heals to get you back to where you want to be. The goal of a therapist is to find things you can do that will develop strength and resilience People are good at self-healing but a therapist can offer expert advice and expertise. The first consultation session is about education, discussing what’s going on and that pain isn’t something to be feared. If you are having a consultation ask as many questions as you need. Know and understand exactly what the plan is It’s about a tailored approach that’s right for each patient. It’s a collaborative approach between the therapist and the patient. BEST MOMENTS ‘To go from having back pain for 5 years to having a pain free walk in 6 weeks is very positive’ ‘It’s meeting the patients and managing their thoughts, fears and expectations’ ‘Movement is what is going to help’ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast recommended products affiliate link ABOUT THE HOSTS Dave Elliot Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Andy Dobinson - A Stroke of Good Luck
Andy Dobinson had a significant and completely unexpected stroke in 2017. His stroke was cryptogenic, meaning the professionals didn’t know why it happened but in this episode, Andy explains not only the event but also his recovery and why he now can say it was the best thing that happened to him. His stroke was a life-changing event for someone who thought they were too young to have a stroke and his recovery changed his perspective building a mindset that is all about the experience, adventure, appreciation and always celebrating the small wins. KEY TAKEAWAYS I had a cryptogenic stroke, meaning they didn’t know why it had happened. In my mind, I was too young for a stroke and I was fit and healthy. Following a stroke, you have to decide to not be a victim and get on with it. My initial goals in my phase 1 recovery were small and incremental. Phase 2 was about getting better and included hydrotherapy and counselling with my focus on getting back to normal. I had to forge new pathways because part of my brain had died. There is fear, anxiety and depression that has to be overcome during recovery. Breaking everything down into small goals and celebrating the little wins is the way to move forward towards bigger goals. The stroke has made me a more rounded human being Its made me appreciate people and things more. If you have a stroke you just have to go with it and see where it takes you. After a stroke, there is a different future for you so you need to go with it. BEST MOMENTS ‘In my mind, I thought young people don’t get strokes’ ‘You have to celebrate the small wins’ ‘I was used to working and training hard so it was a case of applying that mindset in a different way’ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast recommended products affiliate link Andy Instagram @andy_stroke_of_luck NHS signs of stroke https://www.nhs.uk/conditions/stroke/symptoms/ ABOUT THE HOSTS Dave Elliot Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Chronic Pain, CBT, Stoicism and Marcus Aurelius
Donald Robertson is a Cognitive Behavioural psychotherapist and author of ‘How to Think Like a Roman Emperor and ‘Stoicism and the Art of Happiness’ He explains the origins of stoicism and its importance as a philosophy in the world today supporting emotional resilience. As a hugely experienced and knowledgeable CBT psychotherapist Donald also explores cognitive theory and the separation of the emotional response and the physical experience of pain for those who suffer from chronic pain. This is an all-absorbing and fascinating insight into both stoicism and CBT and that has something for everyone who wants to understand more about the ways to take a problem-solving approach to their chronic pain. KEY TAKEAWAYS Cognitive theory is all about the ways our values and beliefs shape our responses. if you can take greater responsibility and ownership for the things you can control then you will be more adaptive and healthier. The language used to describe emotions is very simplistic and this can prevent us from distinguishing between different elements of chronic pain. Thoughts and feelings are intertwined and when you are able to articulate your underlying beliefs and patterns of thinking you are able to begin to ask yourself is this actually true? When you recognise that feelings are thoughts something magical happens that means you can access a huge range of cognitive protocols. If you are focusing on and thinking about a particular pain it can seem amplified For those individuals who believe and are aligned with stoicism, it can offer clients something that CBT or any other type of psychotherapy can’t ever offer them Stoicism can offer something far bigger and far more radical to the individual, a philosophy for life. CBT is diagnosis-specific whilst stoicism approaches things in a more a generic way. In psychology, there is a distinction made between emotional coping and practical problem solving coping. You can either deal with the external problem or how you are responding to it. If you have to live with pain then you may as well learn to live with it wisely. Separating the emotional response is key in being able to problem-solve to cope with pain. BEST MOMENTS ‘When talking to yourself about your pain being as objective and factual as possible is beneficial’ ‘Stoicism is a philosophy but CBT is a therapy’ ‘It’s amazing that this was thought about 2000 years ago and is being used now’ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast recommended products affiliate link Donald website https://donaldrobertson.name Donald’s Courses https://learn.donaldrobertson.name All Donald’s Publications https://learn.donaldrobertson.name/p/publications/ HOW TO THINK LIKE A ROMAN EMPEROR https://amzn.to/33k0Cbm Stoicism and the Art of Happiness https://amzn.to/3nXmSzA

Core Stability & Other Myths
Dave talks to Greg Lehman, Physiotherapist, Chiropractor and Strength and Conditioning Specialist about core stability and the movement that matters in this episode. They discuss what your core actually is and how you can optimise your exercise and movement patterns to maximise the outcomes that are the best for you Greg also shares great information about our, often hidden, 6 packs the different ways in which we exercise them and how everyone has a good core. This is a great chance to understand more about core stability, being human and movement that you enjoy. KEY TAKEAWAYS Your core usually defined as your torso and hips and has many muscles. Exercise has multiple effects and exercise for lower back pain can be helpful but does not have to be very specific exercise. Pain doesn’t always mean you should stop movement but you need to move in a way that is best for you. We should move faultlessly and fearlessly; you shouldn’t have to think you should be able to trust your body. Pain is not just mechanical its influenced by many other factors such as fear and worry There isn’t a relationship between someone’s posture and them having back pain There are joints in your body that enable you to move in different ways so its important that you do just that. You can set yourself ‘goal’ exercises that you want to work towards. Exercise is important especially as we age for bone density and loss of muscle mass For returns on time and energy chose exercise that you enjoy doing so you keep doing it. It’s more about being a human and movement that you enjoy doing. BEST MOMENTS ‘Do whatever makes you feel better because moving in as many different ways as possible will give you more options’ Most people have a 6 pack, you just can’t see it on everyone’ ‘You should be able to move without thought’ VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast recommended products affiliate link OA Optimism website Movement Optimism website Greg Lehman Twitter ABOUT THE HOSTS Dave Elliot Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK.
