I Have Sciatica, Could it be Anything Else?

Sciatica is an overarching term for pain down the back of the leg and is not your diagnosis it’s a symptom that will lead you, with your therapist, down the road to a diagnosis. There are lots of things that can cause the pain and it’s important for the clinician to really understand the history and experience of the patient as part of the process of an accurate diagnosis. This episode looks at some of the many conditions that have pain down the back of the leg and why an individual accurate diagnosis based on the evidence is so important for the patient to ensure the best opportunity for recovery.   KEY TAKEAWAYS Sciatica is an umbrella term for pain down the back of the leg A good health care professional will begin by looking in-depth at your history and experience Specific questioning will help to rule out certain conditions and this will be followed by testing There is a therapeutic value in the professional explaining what they are doing when they are doing it. Radiculopathy is a change in function with pins and needles and a burning sensation Meralgia paresthetica is characterized by tingling, numbness and burning pain in the outer part of the thigh and is caused by compression of the nerves. If you have pain in the back of the legs and pain that intensifies when walking it could be peripheral artery disease. Arteries become narrow and the pressure of the blood causes pain. When you stop exercise, it should improve if you have sciatica the pain will be there constantly. The sacroiliac joint can give a similar pain to sciatica but it is quite rare. Referred pain is when the pain you feel in one part of the body is a result of pain in a different area. If you rapidly increase your running exercise regime you may experience specific pain at the top of the hamstring There are many crossover points between different conditions but all of the conditions have pain down the back of the leg in common Professionals should always be evidence-based and accurate so people have the best opportunity to recover. Giving the person control over their condition will help them to move forward with recovery   BEST MOMENTS ‘Sciatica means different things to different people’ ‘It’s important your clinician explains what they are doing and why’ ‘A good health care professional will let the patient tell their story’   VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ Dr Grimaldi’s  Blog for patients https://hippainhelp.com Dr Grimaldi's Blog for Clinician’s https://hippainhelp.com/dr-alison-grimaldi/   ABOUT THE HOSTS Dave Elliot Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Piriformis Syndrome

Exactly what is piriformis, how is it diagnosed and what you can do about it if you think you have it? All these questions and more are answered by Dr Alison Grimaldi, specialist hip and pelvic pain physiotherapist in this episode that focuses entirely on what piriformis is and how it can be treated. There is lots of confusion about piriformis and the best forms of treatment so this is a great opportunity to understand more about what it is and is not and the best ways to identify the right treatment.   KEY TAKEAWAYS Piriformis is an irritation of the sciatic nerve which usually stems from compression of the nerve as it runs through the buttocks. Piriformis syndrome can be a result of anatomical setup or hypertrophy when the piriformis muscle becomes enlarged through working particularly hard. Signs that you have piriformis syndrome is pain in the mid-buttock area with the pain extending downwards through the leg. Changes in sensory awareness can be an indication of the syndrome Training efficient behaviour of the muscles combined gentle exercise to move the nerve, education and taking away some of the things that are irritating are part of an effective rehabilitation program. Different buttock muscles are affected in different ways by inactivity. A lack of loading on other muscles can change the behaviour of muscles resulting in overactivity by the piriformis muscle. If you are having a cortisol injection combine it with good rehabilitation to make the most of the pain relief and to ensure you have effective strategies in place. Surgery is a last resort option and surgical intervention is releasing the tendon where it joins into the back of the hip so the muscle retracts away from the bone Botox is increasingly being used for piriformis  syndrome For most people, it’s around 3 to 6 months for recovery and there will usually be flare-ups along the way. It’s about re-education of muscle patterns and behaviours. Get a clear diagnosis and pair any injections with good rehabilitation.   BEST MOMENTS ‘Piriformis  syndrome is often diagnosed for any sort of buttock pain’ ‘Cortisol injections can be used as a treatment to give relief although this is usually short term’ ‘Sometimes stretches can be part of the problem, not the solution’   VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast recommended products affiliate link Our Rode Mixer https://amzn.to/3waU8bx Our Microphones https://amzn.to/3rzSZ9Z Second Microphone https://amzn.to/2ObKMeA XLR Cable https://amzn.to/3rBL8ZB Studio Headphones https://amzn.to/3u082LE Laptophttps://amzn.to/3dhfafT Our webcam https://amzn.to/31uUefQ Dr Grimaldi’s  Blog for patients https://hippainhelp.com Dr Grimaldi's Blog for Clinician’s https://hippainhelp.com/dr-alison-grimaldi/ https://hippainhelp.com/recovering-from-piriformis-syndrome ABOUT THE HOSTS Dave Elliot Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Hips and Spines Don't go out of Place!

Many people talk about their hips and spines as being out of alignment, of having ‘slipped out’ and this emotive language has used for some time but this way of describing when something doesn’t feel right can lead to misunderstanding and potentially fear avoidance behaviours. Rob and Dave explain why this language is not only outdated but also incorrect and how manipulation really works along with why it’s about empowering the patient to improve their resilience, have the right information and work in collaboration with their clinician to heal.   KEY TAKEAWAYS When you have pain and your body doesn’t feel right it can feel like something is out of place. Your spinal joints, hip joints and pelvic joints are some of the strongest joints in the body. For these strong joints to be out of place it would take significant trauma on the level of a car crash to cause this. Manipulation, the movement of two joints is equivalent to a mechanical pain killer Manipulation is about creating movement and can be helpful in supporting recovery it’s not about putting something back in place. False beliefs around treatments can lead to fear avoidance behaviour. Clinicians should be empowering the patient to improve their resilience and understand what they can do not what they can’t do. As human beings, we are incredibly resilient. A small change in the language you use can make a big difference   BEST MOMENTS ‘We are designed to move and heal ourselves’ ‘Many people benefit from manipulation and it’s about movement’   VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast recommended products affiliate link   ABOUT THE HOSTS Dave Elliot Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. How Changing Your Mindset will Improve Your Pain

This is a great episode focusing on how the right mindset can make a difference in recovery. What you think and believe about your pain has a huge influence on your potential to recover and a recovery mindset is about managing what you say to yourself, your self-beliefs and updating your education about back pain. If you fully understand what is happening to your body and have a growth mindset you can positively influence your recovery. Look at the bigger picture and understand how to problem solve your way through recovery with a flexible growth mindset if you want to optimise your recovery journey.   KEY TAKEAWAYS You need to fully understand what is happening in your body when you are feeling good when you are feeling pain and what you have the capacity to do. Getting a base level of education is about self-reassurance Fear can be one component of pain-related anxiety Historically people think about their bodies in a mechanistic way but now it would be better to think about the human body as an adaptive and ever-changing biological mechanism You can’t separate the brain from the body it’s all one system and you need to think about things in a more multi-dimensional system. When you move away from ‘it’s one thing’ thinking and look at the bigger picture everything changes. If you have multifactorial back pain you need to be working on all the contributing factors. A fixed mindset is I can’t do that whilst a growth mindset is I can’t do that yet You need to understand the importance of problem-solving your way through recovery Using the 1% approach is about making small changes one at a time that over time make a big change. You should be able to look at things as a combination of approaches. To get into a pain recovery mindset you need to accept that you have pain and where you are starting from.   BEST MOMENTS ‘Update your education so you are empowered and understand the problem’ ‘You need to be flexible in your approach things will go wrong so you have to be prepared to adapt and change’ ‘You need to be optimistic that you can make a recovery’   VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast recommended products affiliate link   ABOUT THE HOSTS Dave Elliot Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Is my Pain From My Sacroiliac Joint?

Mark Laslett is a specialist physiotherapist who has extensive expertise and experience. He explains what the sacroiliac joint is, its function in the body and the varying ways the joint can become inflamed and a cause of pain. He also shares great content on the treatments that are used and the difference between sacroiliac dysfunction and sacroiliac joint pain. This episode is a great opportunity to understand more about the sacroiliac joint and how it can cause pain.   KEY TAKEAWAYS The sacroiliac joint is in the centre of the body between the bottom of the spine and the big bones associated with your buttocks. Its sacroiliac joint doesn’t have any muscles that move it, the muscles that affect it cause it to clamp together. It’s not primarily a movement joint it’s a joint that allows force to transferred from the limb to the spine with a shock-absorbing function. As you age the ability of the joint to move and absorb shock decreases as a normal part of the ageing process. There is a fundamental difference in the sacroiliac joint between male and female with women having more movement which increases during pregnancy. We don’t know what happens during pregnancy but there can be pain in the sacroiliac joint at this time. In the non-pregnant population, the sacroiliac joint almost never gets sprained except when you experience major trauma. There are certain inflammatory conditions that cause pain in the sacroiliac joint but there are good methods for treating these conditions. There is a difference between sacroiliac joint dysfunction and sacroiliac joint pain. If you have true sacroiliac joint pain then manipulation will not make the pain go away.   BEST MOMENTS ‘There is a role for stabilising exercise’ ‘The sacroiliac joint has nerves and this can be a source of pain’ ‘The diagnosis really matters’   VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast recommended products affiliate link   ABOUT THE HOSTS Dave Elliot Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. What is the Best Diet for Recovery?

Ben Steele-Turner is a physiotherapist and associate nutritionist and in this episode discusses what a healthy diet consists of along with the impact of macro-nutrients and micro-nutrients in supporting the essential functions of the body. When a recovery plan and a healthy eating plan are combined recovery is better supported and you are providing your body with the tools it needs to move forwards and Ben shares great content on what makes a healthy diet and why.   KEY TAKEAWAYS If you have a healthy diet then blood flow will be better. It’s about giving the body the tools it needs, are your cells happy, do they have what they need? The building blocks for a healthy diet are - Macro-nutrients – proteins are needed for muscle, bone, haemoglobin and immune cells. Carbohydrates – energy and fuel for cells throughout the day. Dietary fats – for cell activity and hormone activity Micro-nutrients are the vitamins and minerals you need. It’s important to have enough of each micro-nutrient Healthy fats are essential for good health and can be split into saturated and unsaturated fats. Ideally, saturated fats should make up no one than 10% of your diet. Seeds, nuts and avocados are all sources of healthy unsaturated fats An Omega 3 supplement can be taken if you are eating less than the required amount of fruit and vegetables With chronic conditions moving to a more plant-based diet is important in conjunction with the avoidance of highly processed foods.   BEST MOMENTS ‘For your general health sugar intake should be limited’ ‘It’s about everything being in harmony’ ‘When you have exercise help and also have help with healthy eating you are in a better position to self-manage’   VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast recommended products affiliate link Physiutrition Instagram   ABOUT THE HOSTS Dave Elliot Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. How to Get the Most from Your Back Pain Consultation

In this episode, Rob discusses and provides great advice on what you need to do to get the best from your consultation. You need to look for a clinician or therapist who has a skill set that includes an active approach to care, someone who will tell you what you can do to get better and foster self-management, not resilience. Rob shares key information that anyone attending a consultation can implement to make the experience productive and informative. If you have back pain and are unsure about who you should see and what you should expect and ask in the consultation then this is the episode for you.   KEY TAKEAWAYS People who do manual therapy should be making it clear it is a symptomatic relief approach Seek someone who doesn’t undertake imaging as standard for lower back pain. Only a small percentage of people will require imaging and the disadvantages of imaging far outweigh the advantages. Look for someone who encourages self-management not resilience on them The working diagnosis is formed from the information the patient shares about pain and how they are managing. Your family medical history may be explored. You need to be able to explain what the pain is stopping you from doing along with what you want to be able to do. Before undertaking any treatment, you should be asking ’what are the benefits? What are the risks? What are the alternatives? and what will happen if I do nothing? Also, ask about the time frame and the expectations what do they think the outcomes will be for you. Be clear about the plan of action, the prognosis, any lifestyle changes you may need to make and the exercises you will need to undertake.   BEST MOMENTS ‘Make sure they understand what you want to be able to get back to doing’ ‘Any therapist should be fostering self-management’ ‘Make sure they understand what you want to be able to get back to doing’   VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast recommended products affiliate link   ABOUT THE HOSTS Dave Elliot Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Ice, Heat or Peace & Love

There are a variety of factors to be considered when deciding on ice or heat – personal preference, practitioner preference and the scientific data. Rob and Dave share great content on the effects of ice and heat for differing back injuries and explain why people can use a combination along with the importance of early movement to aid positive recovery. Understanding that hurt doesn’t equal harm and how exercise is vital on the journey to recovery along with an optimistic approach is key and in this episode, there is great advice on the best way to tackle the journey.   KEY TAKEAWAYS If its inflammatory or joint pain ice can be effective in keeping pain levels down and aiding movement. If the pain is mostly muscular then heat can be the process that is most effective. The back is different from other parts of the body with the majority of structures that can cause pain located deeper. Protect the injury by stopping the activity and de-loading on the area An excess of inflammation is when we start to experience pain. You don’t want to stop inflammation completely in acute injuries. You need to understand the importance of increasing load gradually. Severe pain does not necessarily mean severe damage Pain is a warning mechanism of the body and hurt doesn’t equal harm. There are psychological factors connected to injury and it’s important to be optimistic about recovery. Revascularisation can be improved through movement and there are huge benefits to early mobilisation.   BEST MOMENTS ‘Exercise is so important for good recovery’ ‘A gradual increase in load and movement is what helps the body to recover’ ‘You can try both ice and heat and see what works for you’   VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast recommended products affiliate link   ABOUT THE HOSTS Dave Elliot Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. Back Pain During Pregnancy 2.0

In this episode Physiotherapist and pelvic health specialist, Sarah Fellows talks about all things pregnancy, pelvic pain and back pain. She discusses the types of pain that may be experienced and one of the biggest drivers for back pain during pregnancy – fear. Training for motherhood is essential and this is a great opportunity to hear about the various factors that impact on back pain during pregnancy and what you can proactively do.   KEY TAKEAWAYS One of the biggest drivers for back pain during pregnancy is fear. Back pain will get better during pregnancy if it is treated well. During pregnancy, there are biomechanical changes with an increasing frontal load. This makes pregnant women more at risk of falling in later pregnancy. Making sure your body is able to move into and out of neutral postures is important. The more babies you have the more likely you are to experience back pain. There are a variety of factors that can contribute to back pain in subsequent pregnancies including nutrition, fear and insufficient rehabilitation from prior pregnancies transfer of load is not so efficient. There are many drivers to pain so a health professional needs to take a detailed history If you have had a traumatic birth then it’s important to take the time to talk to someone and  process it. Make good use of the time after the birth from birth to twelve weeks to process and recover from what’s happened to your body. In pregnancy, structures can become more sensitised causing pain. If you experience transient pain or a decline in function then seek help from a physiotherapist. If you are stressed your breathing is altered and this will affect your pelvic floor. It’s a myth that you shouldn’t exercise when pregnant if you are healthy.   BEST MOMENTS ‘Posture is not an indicator for back pain’ ‘We function on our feet so working out how to move better is key’ ‘It’s important to be as active as you can, the more active you are the better it is for your baby’   VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast recommended products affiliate link Active Pregnancy Foundation Sarah Fellows Instagram   ABOUT THE HOSTS Dave Elliot Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK.
