S1 Ep31: The Truth About Estrogen Therapy and Breast Cancer

The number one reason women give for not taking estrogen is fear of breast cancer. This fear is based on the 2002 study which has been reevaluated and shown to be flawed in a number of ways. Despite solid proof that estrogen therapy does not cause breast cancer and other serious illness, most doctors continue to exaggerate the risks and undersell the benefits of hormone therapy. The goal of this episode is not to convince you to take estrogen. The goal is to present the data so that you can make an informed decision.  In this episode, Dr. Streicher discusses:  The original PURPOSE of the 2002 Women’s Health Initiative (WHI) and how the study was designed.  The INITIAL RESULTS of the WHI study Why the design of the study was PROBLEMATIC Why the results of the WHI were NOT REPRESENTATIVE of the women who take hormone therapy today The results of the RE-ANALYSIS of the WHI How the age of women in the study, the timing of therapy, and the use of statins altered CARDIOVASCULAR data Why the use of PROGESTIN and the kind of progestin in the study changed the results.  Why TRANSDERMAL estrogens have less risk and who should use one The FACTS about the relationship of estrogen and BREAST CANCER The risk of hormone therapy COMPARED to other known risks for breast cancer The medical impact of women NOT taking HT GUIDELINES for taking hormone therapy safely For more information on this topic: Hot Flash Hell: A Gynecologist's Guide to Turning Down the Heat has a more complete discussion of the results of the WHI and includes pros and cons of every type of estrogen currently on the market.  Episode 1: Is It Menopause or Some Other Pause? S1 Ep28: The BEST approach for BREAST Cancer Screening and Risk Reduction with Dr. Lisa Larkin

It used to be so simple. Women were just advised to get a mammogram every year. But now we have not just x-rays, but ultrasound, MRI, digital breast tomosynthesis, screening mammograms, and diagnostic mammograms. Women are confused, and frankly, a lot of doctors are also confused. I am joined by Dr. Lisa Larkin, a board-certified internist, a certified menopause practitioner of the North American Menopause Society, and a nationally recognized expert in breast cancer risk assessment and prevention.  In this episode, Dr. Streicher and Dr. Larkin discuss:  The LIKLIEHOOD of developing breast cancer in different populations WHO is getting MISSED when it comes to early detection  Who should have genetic screening The problem with using over-the-counter testing such as 23 and Me When should someone start -and stop- breast cancer screening What is TOMOSYNTHESIS Who should have an MRI Who should have an ULTRASOUND Need to know info on DENSE BREASTS and why you should look at your report If it matters WHERE someone goes to get their mammogram RISK FACTORS for breast cancer that you can’t change Risk Assessment models to PREDICT your lifetime risk of breast cancer What it means when you are called back for ANOTHER VIEW RISK REDUCING drugs many women do not know about MODIFIABLE risk factors for breast cancer Why ESTROGEN is not a risk factor for breast cancer Why a PLANT-BASED DIET is beneficial Dr. Larkin’s new model for patient care, MS. S1 Ep23: The Impact of Systemic Racism in Mid-life Women's Health

Relatively minimal research is devoted to menopause even though menopause affects literally 100% of women that live past the age of 50. For women of color, the situation is far more dire.. Not only do we know less about the impact of medical conditions or therapies in mid-life black women but it is incredibly challenging for many black women to access appropriate care. My guest is Dr. Fenwa Milhouse, a urologist who is not only a talented surgeon, but uses social media to destigmatize pelvic floor & sexual conditions, as well as demonstrate the importance of representation in medicine. Dr. Streicher and Dr. Milhouse discuss: DISEASES that disproportionately affect Black women How HYSTERECTOMY impacts cardiovascular disease in Black women Why Black women are less likely to take HORMONE THERAPY BLACK CELEBRITIES and increasing awareness of health conditions BARRIERS to health care in Black underserved community Micro and Micro AGRESSIONS in the health care system The reason for and impact of IMPLICIT BIAS The challenges of a BLACK WOMAN UROLOGIST practicing in a white hospital The importance of DIVERSITY EQUITY and INCLUSION initiatives in our health system What impacts racial disparity and potential action steps to improve health in the Black community How SYSTEMIC RACISM impacts on health Dr Fenwa Famakinwa Milhouse is a fellowship-trained, board certified urologist. Originally born in Nigeria, her family immigrated to the US when she was a young child. She grew up in Texas and ended up at the University of Texas at Houston for medical school. She trained at the University of Chicago for residency and then completed a fellowship in female pelvic medicine and reconstructive surgery. She is a wife and mother of 3. Dr. Milhouse is passionate about introducing the public to the world of urology, mentoring underrepresented minorities, and diversity and inclusivity in the workplace. She uses social media to destigmatize pelvic floor & sexual conditions, as well as demonstrate the importance of representation in medicine.  IG: @drmilhouseTIKTOK @yourfavoriteurologistTwitter: @FMilhouseMDWebsite: www.yourfavoriteurologist.com The Black Women's Health Imperative- nonprofit organization created by Black women to help advance the health and wellness of Black girls and women.https://bwhi.org/Lauren Streicher, MD is a clinical professor of obstetrics and gynecology at Northwestern University’s Feinberg School of Medicine, and the medical director of the Northwestern Medicine Center for Sexual Medicine and Menopause. S1 Ep23: Welcome to Peri-menopause!

Years before estrogen production completely shuts down, HORMONE LEVELS start to FLUCTUATE wildly during that special time of life known as PERIMENOPAUSE. Waiting out bothersome hot flashes, mood swings, and insomnia is not generally the best strategy since perimenopause can last for years. In addition, it’s not as if those symptoms magically go away when you enter menopause!   In this episode Dr. Streicher discusses: WHEN perimenopause typically starts The difference between SYMPTOMS of perimenopause and menopause SOLUTIONS to cope with perimenopausal hormonal fluctuations If taking BIRTH CONTROL PILLS is a good idea or a bad idea  HOW LONG you can safely take birth control pills The difference between birth control pills and MENOPAUSE HORMONE THERAPY (MHT) Why it is a good idea to take birth control pills CONTINUOUSLY When to start MENOPAUSE HORMONE THERAPY Further information on perimenopause can be found in Hot Flash Hell: A Gynecologist's Guide to Turning Down the Heat Lauren Streicher, MD is a clinical professor of obstetrics and gynecology at Northwestern University’s Feinberg School of Medicine and the medical director of the Northwestern Medicine Center for Sexual Medicine and Menopause. S1 Ep20: Is Non-monogamy Having a Moment? With Dr. Wednesday Martin

Prepare to rethink everything you have been taught about female sexuality. One in five couples practice open non-monogamy, yet cultural and societal expectations are that a woman have only one partner for life. Dr. Martin’s book, UNTRUE: Why Nearly Everything We Believe About Women, Lust, and Adultery is Wrong-and how the new science can set us free “cuts through the junk science and regressive cultural narratives that have shaped our beliefs about female infidelity for centuries revealing, a truth both liberating and disconcerting: women are no more “naturally monogamous” than men; nor are their libidos shrinking violets.” In this episode, Dr. Streicher and Dr. Martin discuss: WHY the science of female sexuality is mis-represented, misunderstood, and usually wrong Why there is so little INFORMATION on the CLITORIS If MEN are biologically MORE SEXUAL and have higher libidos than women The difference between spontaneous and RESPONSIVE DESIRE The ORGASM GAP! Why female pleasure is such a low priority What libido DRUGS are really medicating Why the clitoris is “forward facing” instead of inside the vagina Lessons learned from primate sexual behavior, and what animal to visit next time you go to the zoo Why from an anthropologic point of view, woman are not BIOLOGICALLY programmed to have ONE PARTNER When and why women LOSE INTEREST in sex with a long-term partner, assuming that the sex is still worth having  Why monogamy is harder for women than it is for men If women CHEAT as often as men  All about “OPEN NON-MONOGAMY” What it is, how many people are monogamous, why the numbers are growing and how to broach the topic with your partner   If JEALOUSY is inevitable with openly non-monogamous couples Where in the world is the BEST PLACE to be a woman sexually For more information on Dr. Wednesday Martin, visit: WednesdayMartin.com Dr. Martin on Instagram: @WednesdayMartinphd Dr. Martin’s Books: UNTRUE: Why Nearly Everything We Believe About Women, Lust, and Adultery is Wrong-and how the new science can set us free Primates of Park Avenue Stepmonster Lauren Streicher, MD is a clinical professor of obstetrics and gynecology at Northwestern University’s Feinberg School of Medicine, and the medical director of the Northwestern Medicine Center for Sexual Medicine and Menopause. S1 Ep17: Menopause and Work

More than 60% of women say that their menopause symptoms decreased their productivity and quality of work and even though millions of corporate dollars are lost due to absenteeism, reduced quality of work, and health care costs, the US government and corporate America refuse to recognize or mitigate the problem. The time is way overdue to acknowledge and fix it.  In this episode Dr. Streicher discusses:  The PERSONAL and ECONOMIC impact of menopause in the workplace RESEARCH on how often specific symptoms impact specific aspects of work WHY women do not talk to their bosses about menopause symptoms How the UK addresses and REDUCES the impact Why the US needs to do what the UK is doing  UK website on Menopause in the workplace: cipd.co.uk/menopauseforHR To learn more about how hot flashes can impact productivity and longevity, take a listen to Episode 2:Think Your Hot Flashes Can't Kill You?
