S1 Ep63: Are Current Screening Guidelines HELPING women or HURTING Women? Controversies in Public Health Policy with Dr. Melissa Simon

Today I am talking to Dr. Melissa Simon. Dr. Simon served on the US Preventative Services Task Force Recommendation Committee (USPSTF), the government organization that shapes public policy and sets recommendations followed by doctors all over the country. So, when your doctor says you only need a Pap test every 3 years and that you no longer need a Pap test at all after age 65,  it is based on the recommendation of this committee.  How do they come up with these guidelines? And, are they always right? In this episode I challenge Dr. Simon on some controversial Task Force recommendations that have informed the way your doctor practices medicine, and dictated the health care you receive. And if you listened to Episode 58 Two Experts Two Timely Topics, you know what an amazing guest Dr. Simon is!  We discuss:  The role of the National Institute of Health (NIH)  The role of the National Institute of US Preventative Services Task Force Recommendation Committee (USPSTF) Problems with the current Pap test recommendations If women over 65 should get a pap test, and if so, who would benefit Who should get genetic testing for the BRCA mutation  Why urinary incontinence is a public health issue The public library and public health How you can influence public health policies  US Preventative Services Task Force Recommendation Committee https://www.uspreventiveservicestaskforce.org/uspstf/ Health for All Library Project: https://healthforallproject.org/  To leave a comment on cervical cancer screening: (comments periodically close but often open up again) https://www.uspreventiveservicestaskforce.org/uspstf/draft-update-summary/cervical-cancer-screening-adults-adolescents  Final plan for cervical screening  which will also post for public comment: https://www.uspreventiveservicestaskforce.org/uspstf/document/final-research-plan/cervical-cancer-screening-adults-adolescents To find out what preventative services you should have: https://www.uspreventiveservicestaskforce.org/apps/                                                   Melissa Simon MD Twitter: @DrMelissa Simon LinkedIn: 2Melissa-simon https://www.feinberg.northwestern.edu/faculty-profiles/az/profile.html?xid=16856 For more information: Episode 26: Diversity, Equity, and Inclusion  in Health Care, and Why it Matters with Dr. Fenwa Milhouse Episode 39 A Personal Trainer for Your Pelvic Floor with Dr. Janelle Howell  Episode 44 A Deep Dive into RECURRENT UTI Episode 58 Two Experts Two Timely Topics Lauren Streicher, MD is a clinical professor of obstetrics and gynecology at Northwestern University’s Feinberg School of Medicine, and the founding medical director of the Northwestern Medicine Center for Sexual Medicine and Menopause. S1 Ep56: Too Young for Menopause?

Every day women tell me they were blindsided when they started having perimenopause symptoms in their mid-40’s since they assumed that menopause was years, or even a decade away. And they are pretty much shocked when I tell them that mid-forties is the normal time for estrogen levels to waver, or tank all together. But imagine if you enter menopause when you are in your 20’s or 30’s?  This situation is more common than most people appreciate occuring in up to 3% of women.  In this episode I am joined by Dr. Hannah Short, a graduate of the University of Cambridge and a specialist in premature menopause recognized by the British Menopause Society. She is the author of The Complete Guide to POI and Early Menopause.  We discuss:  The difference between EARLY MENOPAUSE, PREMATURE MENOPAUSE and PREMATURE OVARIAN INSUFFICIENCY, otherwise known as POI  WHY Premature Menopause occurs Why POI occurs The impact of ENVIRONMENTAL factors Why SMOKING increases the risk of premature menopause Why it is so important to DIAGNOSE and TREAT women who enter menopause early If HORMONE THERAPY is prescribed in different doses or in a different amount than for women who go through menopause at a typical time HOW LONG women should take hormone therapy NON-HORMONAL options for women who cannot take hormone therapy                                                     Dr. Hannah Short Website: https://drhannahshort.co.uk/  Twitter: @drhannahshort The Complete Guide to POI and Early Menopause GoodReads: https://www.goodreads.com/book/show/60382898-the-complete-guide-to-poi-and-early-menopause? S1 Ep51: Transdermal vs Oral Estrogens- What’s the Difference?

Systemic estrogen, unlike local estrogen, affect every single cell in the body which means there is an impact on not only hot flashes but also your bones, your brain, your bladder, and your cardiovascular system along with your vulva and vagina. While there is a long list of benefits with both oral and transdermal systemic estrogen, the potential complications are where the differences come in. So, the decision to take an oral estrogen versus using a patch, spray, or skin gel is not just about personal preference, insurance coverage, and convenience, it’s also about safety. It’s about potential risks. It’s about your personal medical history. In this episode, Dr. Streicher discusses:  The BENEFITS of oral and transdermal systemic estrogen The DIFFERENCE between oral and transdermal estrogen RISKS that are specific to ORAL estrogen products BENEFITS that are specific to ORAL estrogen products BENEFITS that are specific to TRANSDERMAL estrogen products Why the FDA label lists risks for estrogen products that are NOT TRUE TRANSVAGINAL systemic estrogen PROS and CONS of specific transdermal estrogen products Estrogen PATCHES  Estrogen GELS  Estrogen SPRAY  TOP TEN QUESTIONS my patients ask about transdermal estrogen For more information on this topic:  Hot Flash Hell: A Gynecologist's Guide to Turning Down the Heat Links to other episodes on this topic:  Episode 2:Think Your Hot Flashes Can't Kill You?
