Dr Benjamin Bikman | 3 Ways to Support Your Kidneys, Effect of Salt on Insulin Sensitivity & More! KKP: 259

Today, I am blessed to have here with me for a THIRD time Dr. Benjamin Bikman. He is a biomedical scientist, professor, and expert on insulin resistance. Dr. Benjamin Bikman studies diabetes and obesity, and much of what he focuses on is a seemingly obscure condition called insulin resistance. But it's, in fact, not very obscure at all.  What is insulin resistance? Why did Dr. Benjamin Bikman devote his career to studying something like this? Well, insulin resistance is essentially two things. First of all, some cells in your body aren't responding very well to the hormone insulin. Second, you have more insulin than you did before, a condition called hyperinsulinemia. Dr. Bikman's research focus is to elucidate the molecular mechanisms that mediate the disruption that causes and accompanies metabolic disorders, such as obesity, type 2 diabetes, and dementia. Driven by his academic training (Ph.D. in Bioenergetics and postdoctoral fellowship with the Duke-National University of Singapore in metabolic disorders), he is currently exploring the different roles of insulin and ketones as critical drivers of metabolic function. He frequently publishes his research in peer-reviewed journals and presents at international science meetings. In this episode, Dr. Bikman starts by telling us about his journey with social media. In this, he explains his thoughts on why people are so addicted to hatred. He then describes his new manuscript about the differences in the hippocampus of Alzheimer's brains vs. control brains. Next, he discusses sphingolipids, different types of fat growth, and the importance of our humble kidneys! Most Humans Struggle With This There are chemical mediators to the emotions that we associate with hate. As much as we want to say we hated something in the heat of the moment, the feeling is somewhat satisfying. It is like we've just sunk our teeth in, and we've made the kill. There's almost an animalistic instinct, a satisfaction that comes from letting someone know what you think of them and expressing your anger, hatred, or frustration at them. That is an addictive state. [16:00] Differences In the Hippocampus of Alzheimer's Brains vs. Control Brains Bikman explains that in a new study, they were able to obtain samples of the hippocampus from donors who had passed away both with and without Alzheimer's disease. The hippocampus is the learning memory center of the human brain. When comparing the control brains and the Alzheimer's brains, they found a profound reduction in almost every gene involved in glucose metabolism in the Alzheimer's brains. However, when looking at ketone metabolism, both the control and the Alzheimer's brains looked totally normal. This is a massive step in Alzheimer's research because it shows that it is not a plaque disorder as people believe; it is a metabolic disorder.  [19:20] The Effect of Sphingolipids In The Brain – A Mystery Solved There is a class of fats in our cells called sphingolipids. They were named after the mysterious sphynx because we didn't know what they did for so long. However, now we know sphingolipids are doing two things. The first is causing the hippocampus to become insulin resistant. Which in turn results in the reduction in glucose-related genes being expressed. The second is sphingolipids directly alter the function of mitochondria. Mitochondria produce most of the cell's energy, so if they can't function well, neither can the cell. [26:34] Hypertrophy vs. Hyperplasia Fat Growth Fat tissue, what we pinch and jiggle, grows in two different ways. Hypertrophy is where the number of fat cells is set, but each fat cell itself gets bigger. That is a finite growth because once the fat cell has reached the maximum dimension, the fat tissue stops growing. An alternative mechanism of growth is hyperplasia, where the fat cell gets a little big, and then we make another fat cell and then make another fat cell, and it continues. Hyperplasia is essentially limitless fat growth. However, that's actually a healthier way to get fat. If you can grow fat cells through hyperplasia, they stay very insulin sensitive, and the body stays insulin sensitive. [34:00] The Humble Kidney: It’s A Tissue That’s In High Demand! Bikman was surprised in his research at how high the metabolic demand is in the kidney. In fact, he says he's never seen a tissue with such high demand as the kidney. Compared to the heart, which is known to have high metabolic demand, the kidney blew through oxygen much faster! People look at the humble kidney and think that it is just filtering blood, when in fact, it is doing much more. Of course, it also regulates the hormones responding to insulin. But even further, it is actually what sends the signal when the body is getting low on oxygen. [37:58] Three Ways We Can Support Our Kidneys One of the most important ways to keep our kidneys healthy is to stay hydrated. Second, Dr. Bikman emphasizes the importance of learning to breathe better to increase blood flow through the body. One way to do this is by using strategic vasodilation with brief breath holds from time to time, especially while moving. Last, keeping your insulin low is also important for kidney health. If insulin is high, then insulin is forcing aldosterone to be high. Then aldosterone tells the kidneys to hold onto all this stuff that we don't want to hold onto. [42:59] Has Anybody Ever Died From Type Two Diabetes? Diabetes is considered the seventh leading cause of death. Type two diabetes is overwhelmingly the most common form of diabetes. For someone to die of purely type two diabetes, the one thing that might kill someone is a non-ketotic coma. This occurs when their glucose levels are so massively high that they become hypovolemic. This means their blood pressure would be so low that they cannot pump blood to their brain. This can cause them to lose consciousness and potentially die. However, this is extremely uncommon. What usually kills people with diabetes is other health issues accelerated by diabetes, such as heart disease or cancers. [47:18] The Effect Of Salt On Insulin Sensitivity May Surprise You The obvious way salt and insulin sensitivity are related is that when insulin is up, it pushes aldosterone to be up, which forces the kidney to hold onto more salt and water. More water means higher blood volume, which in turn means higher pressure, leading to hypertension. However, if you restrict salt, the body becomes determined to hold onto any salt that it can. This makes the body insulin resistant. The body makes itself insulin resistant in this scenario because it will raise insulin, which will raise aldosterone, making the body retain salt. So, when you think about today's often dietary advice to cut salt, they are really creating this vicious cycle by exacerbating the insulin resistance. AND MUCH MORE! Today, I am blessed to have here with me for a second time, Dr. Nasha Winters. She has been on a personal journey with cancer for the last 27 years. Her quest to save her own life has transformed into a mission to support others on a similar journey. Dr. Nasha travels the world to explore integrative cancer clinics, vet cancer protocols for research projects, speak at conferences, and meet with colleagues to help them apply metabolic approaches with their patients. Dr. Nasha offers doctor-to-doctor consultations and intensive mentorships for clinicians who want to more deeply understand metabolic and integrative approaches to cancer. Even though we intellectually understand death, it's definitely still very painful. At 19, Dr. Nasha met her own diagnosis. It took a long time for Dr. Nasha to really come to terms with understanding death and dying. However, It's not something we're taught in our culture. Yet when you work with chronically ill people with cancer, you're going to see it, and you're going to see it often. Nasha learned to understand that death and dying look exactly the same. They're just an unbelievable passing through a particular threshold; they are not as scary and ugly as we've made them out to be in our world. Death is a powerful threshold; it is a very normal process of life. [09:40] What We Can Do To Extend Our Lifespan Our fountain of youth resides in the health and wealth of our mitochondria. Nasha explains that, unfortunately, new beings are entering this planet with their mitochondria already somewhat polluted and damaged. Then, the longer you are on this planet, and the more you neglect your health, the more damage you do to your mitochondria as well. These mitochondrial conditions make you age faster and be more susceptible to chronic illness processes. Nasha emphasizes that preventative care is key to heading off these conditions and extending our lifespan. There are many easy ways to access what is actually happening in your body, and as a culture, we need to start caring about not just feeling fine but being optimally healthy. Whether people like it or not, this "optimal health" is heavily influenced not only by physical factors, but by our life experiences, emotional intelligence, resilience to stressors, and other important mental, emotional and spiritual aspects of our life. [21:12] Metabolic vs. Somatic Theories of Cancer Explained By Dr. Nasha Somatic theory is the concept that our DNA accumulates damages to its structure over time and then sets off a path to a cancering process. This theory supports the idea that cancer is a “bad luck game” and you’re just either going to get it or you’re not. Nasha does not subscribe to this thought process. It creates the idea that you are powerless, and powerlessness is an incredible toxin. Metabolic theory instead conceptualizes that we can see where the cancer cells stem from and that we can make a difference. From our thoughts to our food, to the water we drink and the air we breathe, everything around us affects our mitochondria. Our mitochondria are the “genome protectors;” the key to warding off cancerous cells. So keeping them healthy is crucial. This is where Dr. Nasha sees hope in the vision of cancer, that we can make a difference. [28:12] What Is Affecting Cancerous Cell Replication + What You Can Do About It Once an unhealthy cell switches its fuel source to sugar and starts replicating, it will just keep replicating. Our ability to break to kill off those damaged, dysfunctional or rogue cells is under the complete guidance of our mitochondria. So if our mitochondria are unhealthy, damaged, or inefficient, they cannot stop the attacks. But if you get your body fat down and build up your muscle, you actually start to increase your storage and effectiveness, and efficiency of your mitochondria. Nasha has found that she can help patients take the mitochondria they have and strengthen them through powerful tools like intermittent fasting, cryotherapy, and more. Unfortunately, the pharmaceutical industry can’t get rich off these tools, so they push it to the back. All while we keep trying to find the pharmaceutical answer to the ancient wisdom that's been available to us all along. [37:47] The Importance of Metabolic Flexibility Nasha has found that the vast majority of her patients are not very metabolically flexible. Most people in their normal lives tend to eat something every 2 to 3 hours and also like to indulge in a little snack or sweet treat between dinner and bed. However, learning to fast can be extremely important. In a 1909 study, it was found that simply fasting cytotoxically reduced the size of a tumor. So, if you take fasting and pair it with chemo, you get a synergy, like a double whammy. All while protecting those healthy mitochondria from the damaging effects of that chemo or that radiation. Yet, most oncologists still aren't full-heartedly supporting their patients fasting during chemo. [46:23] How Exogenous Ketones Help During Radiation Nasha emphasizes that just taking exogenous ketones will never be as good as doing ketogenesis the old fashion way. However, exogenous ketones can help to give a running start to people who are new to low carb or need to get primed for a particular chemotherapy. They also help reduce the withdrawal from carbs that most patients feel at first, giving your psychology time to catch up with your physiology. Exogenous ketones can even pack an extra punch for folks who are already fat-adapted during radiation to protect the healthy tissues. This is also important for patients that have brain tumors, for instance, because the steroids they might be taking make it almost impossible to get into therapeutic ketosis. Another great way to get into ketosis is a three-day water fast. Those doing this around their chemo find exogenous ketones keep them feeling pretty good. [50:18] What Level of Ketones Are Optimal For Radiation + Therapeutic Ketosis Ideally, Dr. Nasha wants them in therapeutic ketosis, which is above three on the Keto-Mojo. For people to get into a level of three and higher, you have to be very metabolically flexible and/or taking exogenous ketones. What gets someone to a certain level of ketones is unique to the person. Not only are bodies unique on their own, but all the stress and different medications can make it even harder to get into ketosis during cancer treatment. The point being, don't beat yourself up if it is harder for you to get into ketosis than someone else. [53:40] Dr. Nasha’s Favorite Inflammatory Markers Nasha’s trifecta: C Reactive Protein (CRP), Sedimentation Rate (ESR), and Lactate Dehydrogenase (LDH) CRP is prognostic for many things. If you have an elevated CRP at the time of your breast cancer diagnosis or colorectal cancer diagnosis, you have a poor prognosis. Sedimentation rate tells you how fast your blood cells fall out of plasma. If the cells fall out fast, that is good; you want flow in your blood. If they fall out slow, your blood is more gelatinous and also means you have more inflammation in your tissues. LDH is the most important test for your metabolic health. If your LDH is high, your mitochondria are off. When using this trifecta as a collective, they are more sensitive and specific than any other cancer marker Dr. Nasha has seen. AND MUCH MORE! But the truth is, our daily eating schedule—three meals a day, plus snacks—does not necessarily reflect our bodies’ natural cycles. Cole’s book is all about intuitive fasting. However, fasting will be anything but intuitive when you're metabolically inflexible. Instead, the book is a conversation about showing up for yourself to become metabolically flexible. When you are finally metabolically flexible, you will intuitively fast and eat. You can mindfully use intermittent fasting to gain metabolic flexibility so you can hear what your body truly loves.   [05:25] Why Dr. Cole Is A Huge Fan Of Fasting For His Health Fasting is an amazing tool, and it's completely free. Plus, fasting is something that it's actually encoded in our DNA. Researchers estimate that our genetics haven't changed in 10,000 years. Your body is made to do so much. It’s time to wake up the body's ability to heal itself and wake up the body's ability to calm inflammation. Sadly, we are living so out of alignment with our biochemistry that we put a lot of our body's healing capacity just to sleep. Fasting is a matter of letting our bodies remember what we were born with. Overall, fasting is an amazing tool that clinically really improves people's labs. [09:25] How Fasting Can Actually Heal Your Gut + Reset Your Digestive System Your microbiome is actually repairing through the night and recalibrating through the night. When you fast, it is calming down the inflammatory cascades that are going on. Also, fasting doesn’t mean that you are cutting calories. Instead, you're eating the same number of foods, you're just eating in specific windows to give your body a little bit more of a fasting time than you normally would. The more often you can fast, you're going to be able to regenerate and repair the gut in a big way. Digesting food requires a lot of energy; so, you're giving your gut a break to not work on digesting food. [13:45] What Food Freedom Means To Dr. Cole Food peace is going longer without eating, not because it's restrictive - but because you can do it, and you feel great doing it. Also, food peace means you can break the fast when you want to. Intuitive fasting can be scary; it can be triggering for people that are addicted to food. Just because something's common doesn't necessarily mean it's normal – most people in the United States have a massive blood sugar problem. The traditional way that American’s live their lives are, in many ways, self-harm; it's not self-respect at all. Food freedom is integrating things in your life that make you feel great. [17:55] The Combination Of Keto And Fasting Is A Total Game-Changer You can’t fast your way out of a poor diet. Dr. Cole doesn’t advocate for people to have these extreme approaches to fasting, where it's like an eating disorder disguised as a wellness practice. Fasting should be a tool that can actually heal your relationship with food, heal your relationship with your body, and actually heal your body. If you look at a lot of the research on intermittent fasting and the ketogenic diet, you're going to see similar pathways explored because they're both supporting beta-Hydroxybutyrate. Keto is nutrient-dense and calorically rich foods that are very satiating and nourishing; it's going to make your intermittent fasting window easier. [24:00] Can You Teach Me How To Use Fasting To Lose Weight? Many people hear that fasting is connected with weight loss. However, it's essential to realize that fasting is the body repairing things. If you want to fast to lose weight, you need to shift your mindset. Instead, you have to get healthy to lose weight, not the other way around. Fasting has a lot more benefits than weight loss – you should be more excited about those aspects of fasting. [28:05] Dr. Cole Reveals His Favorite Supplements For Fasting Cole is a fan of having coffee and tea while he fasts. Earl Grey tea has been shown to support our autophagy pathways. The main supplement you’ll want when fasting is electrolytes. Again, you can’t fast your way out of a poor diet. If you’re eating clean foods, you may not need those electrolytes. You’re Not A Failure If You Cut Your Fast Early When fasting, you need to have flexibility and intuition. If you cut your fast early, it’s not a failure. Instead, it’s a gift that you listened to your body. Changing your fasting plan is part of metabolic flexibility. We think there has to be this rigid plan for fasting to be successful; however, that’s not how life works! [38:05] Stop Comparing Yourself To Other People On Your Health Journey We're all on our own journey with health – stop comparing your journey to another person. We're all created differently, and we're all up against something differently. Obviously, we're learning from other people. However, your path will be different. Communities should be there for insight rather than comparisons. You're on your own journey, you're going to do great, but in your own way. [43:55] The Truth About Autophagy: Are You Getting Too Much While Fasting? Constantly check in with yourself and see how you feel: Are you burnt out? Are you fatigued? Do you feel like you’re starving? As long as you eat enough foods during your eating window, then you should be okay. However, if your body is saying otherwise, then eat! Sadly, the food industry has figured out that the reward pathway will keep people coming back for their food. Brilliant scientists are paid good money to figure out how to activate those pathways. [08:50] The Negative Impacts On Your Brain When Consuming Sugars Sugars will light up part of the brain that’s emotional and impulsive. It’s the part of our brain that makes us do things without thought - this is all subconscious thinking. Plus, sugar will trigger dopamine primarily, which is our neurochemical of desire. There's also endorphins and serotonin. Endorphins trigger comfort while serotonin triggers connectedness. Addiction will make you want an entire bag of chips. You can’t stop at one or two because your neural biology is so damaged. The physical manifestations of sugar and high-carb addiction are happening in the brain. [17:00] What You Need To Know About The Different Stages of Food Addiction When we talk about food addiction, it's essentially like diabetes. Food addiction is a chronic and progressive condition. You may have a food addiction if you would rather stay at home and eat instead of going out for a social occasion. If you consistently do not feel well because of the food you eat, you may be suffering from food addiction. Another sign of food addiction is eating foods that you know are not going to make you feel good. If your obsession with food is excessive, then you are a food addict. Lastly, if you are in trouble with your health, then you may be a food addict. “You have to be very careful if you're moving down this path of food addiction.” -Dr. Vera [25:25] Fasting For People With Addictions To Food Cutting out sugar will be the first step for people who have an addiction to food. Sugar is the trigger – so, ditch the trigger! If you have a food addiction, be aware of pitfalls. If you have a history of starving yourself, then fasting may not be the right path. Fasting can be healing; however, it’s a tool that can hurt you if you do not know how to use it. If it’s done carefully, fasting can be great for the body. [30:40] Why Dr. Vera Is A Massive Fan Of Eating More Protein Vera isn’t necessary a fan of protein. Instead, Dr. Vera is a fan of low-carb. Protein and fat will give the body satiety. Most likely, if you are low-carb, then you are eating higher protein. Vera says she doesn’t have a problem with carbs as long as they are not processed. [32:15] It Takes Three To Four Weeks To Kick Your Sugar Addiction It takes about three of four weeks to quit an addiction fully. A connection with a daily coach or sponsor will be critical on your journey. So, find support from someone while kicking your addiction. At the hospital, they gave her morphine. Sadly, Dr. Warren had developed thousands of stones in her gallbladder. After getting the gallbladder removed, Dr. Warren needed to recover from morphine withdrawals. Even at 16, Dr. Warren wondered why she was born with a gallbladder if she didn’t need it. Just because they cut out the gallbladder, that didn’t mean Dr. Warren was cured. At 19, Dr. Warren found out that she had cancer. However, it was the “good” cancer to have, thyroid cancer. Well, the doctors were ready to have her thyroid cut out, and Dr. Warren had the surgery done. Warren became weight loss resistant, and she started to struggle with depression. When you think about depression, you think about suicide. However, depression can mean waking up every single day with a massive burden. Eventually, Dr. Warren found keto and fasting to change the healing process in her body. [10:30] An Overview Of The Thyroid + What To Know About Reverse T3 The hypothalamus is a thyroid-releasing hormone; it goes from the hypothalamus to the pituitary and the pituitary to the thyroid. When we talk about carbs, fat, and ketosis, it all goes back to the hypothalamus. The hypothalamus is the master control center of all the hormones, not just your thyroid hormones, but all hormones. The Reverse T3 (RT3) Test is one that Dr. Warren recommends. Many doctors have an issue with it. RT3 is an inflammatory marker that gives you a good idea of how your body responds to stressors. We can’t wait around for studies to prove what we know or feel in our body. You have to trust your body because you live in your body. [18:10] Can You Do Keto And Fasting With A Thyroid Issue? Listen to what your body is telling you to do. Getting in ketosis can decrease free T3 – which isn’t necessarily a bad thing. People think they need carbs for free T3. However, you make more free T3 to deal with inefficiencies when it comes to metabolism. Your blood work might show that your free T3 is low, but your body knows that it doesn’t need it as much. Remember, the hypothalamus will regulate what's going on within our body. The thyroid is a layered thing. When it’s off, it will affect loads of different things. When you utilize keto, it will help the gut microbiome. Then, they will need fewer medications. On day three of a block fast, people will get an increased heart rate, and their whole body will change. When you start fasting, you will become more sensitive and need less medicine. Having adverse reactions is a good thing! It means your body is changing, and it’s getting better. Also, fasting can affect antibody levels because of what is happening in the gut. Plus, fasting is great for the immune system. [35:15] Why It Is Dangerous To Stay In Ketosis Long-Term For Thyroid Function   The hypothalamus will control all of your hormones. Ketosis is a stressor that will heal your body. However, it’s still a stressor. When we stay in sympathetic for too long, we get signals of survival. If we are in ketosis for too long, we will go into survival mode. When we are in survival mode, our thyroid will shut down. Having flex days will tell your body that there is no stressor. Now, your body can utilize your carbs to produce hormones in your thyroid. [42:50] Finding Doctors + Health Practitioners On Your Healing Journey Find someone who is going to respect you and someone who is going to have a conversation with you. Those heavy metals are in big enough chunks that your immune system cannot clear them. Sunshine will zap the large chunks of heavy metals. Total Cholesterol And All-Cause Mortality There’s a study called: Total cholesterol and all-cause mortality by sex and age: a prospective cohort study among 12.8 million adults. You can check it out here: https://www.nature.com/articles/s41598-018-38461-y The research is very clear about optimal levels for total cholesterol. When you get above 200 on your cholesterol, doctors will tell you that you need statins as soon as possible. Jay takes the prescriptions and then throws them out. However, if your blood sugar's above 85, fasted, that's a threefold higher risk of heart disease. Anything above 85 is not considered optimal. Americans will come in at 95, and a cardiologist will ignore that. [16:30] The Optimal Ranges For Your Vitamin D Levels 30 – 70 or 30 – 100 are optimal ranges. The average American is at 30. As soon as you get below 50, you see associations with health issues. For instance, people will suffer from gut issues, chronic fatigue, and depression with those levels. If you are at 200, there is some risk of calcifying your arteries. Keep it between 50 and 100 for optimization. Getting in the sun will bump your levels up to 70 or 80. [19:50] The Issues With Sunscreen - How Safe Is Sunscreen? The main ingredient is oxybenzone. It’s a manmade chemical, and it acts like estrogen in your body. You do not want to rub it on your skin. After one application of sunscreen, oxybenzone levels were above the government's safety levels even one week later. The government limits are absurd, anyway! Estrogen chemicals don’t kill the cells because they act like estrogen. It will do a lot of weird things to the cell instead. Zinc is an excellent alternative for sunscreen. Most people are sleep-deprived. Jay will get an hour and a half of deep sleep and an hour and a half of REM sleep. In college, Dr. Jay would sleep three hours a night. He would brag about it! Infrared lights will help you sleep way better. Also, the colder the bedroom, the deeper sleep you will get. Intermittent fasting will also help with your immune system. That way, your cells can take out the trash. Sunshine is another way to improve your immune system. Don’t forget to get your skin in the sun – take those shirts off! [36:45] Why You Might Want To Think Twice About The Covid Vaccines When researchers do all these studies for pharmaceutical companies and come out with a drug, they don't actually have to release that study. When someone has an adverse reaction to a drug, the company has to release the study. With vaccines, the companies do not have to release the studies. It will cause you to burn excess body fat and improve your overall health. But there’s one important caveat. A constant, unrelenting state of ketosis—in which your body is burning its own fat for fuel—is obviously unsustainable, because eventually you’d have no more fat. In this chapter, we move into the Flex pillar, which will allow you to maintain your Keto lifestyle month after month, year after year. Four Reasons Why Prolonged Ketosis Is Counterproductive My mantra is, “Lose the excess fat; treasure the necessary fat.” Ketosis is a brilliant strategy for accomplishing the first goal. As for the second goal, prolonged ketosis doesn’t have an automatic shut-off valve. If you remain in state of constant ketosis, burning your own fat, then your body will keep doing that as long as it must to stay alive. If necessary to preserve life, once your fat is gone, various bodily functions will be shut off or minimized. This process of shutting down will continue until Mr. Death knocks on your door and says, “Sorry, buddy—time’s up.” No one wants that. To ensure that you’re well informed, let’s review some of the bad things that can happen if you go too far with ketosis. Fat Burning Slows Down As levels of body fat drop, your primal brain, whom we thought we had trained to accept a high-fat diet and fasting, will be aroused and will begin to object. Why and when will this happen? Your primal brain, that eternal watchdog, will sense that your reserves of fat are becoming depleted. Remembering those long periods of deprivation thousands of years ago, your primal brain will sound the alarm: The fat supply is running low! It will issue the command that your body must preserve its precious fuel. In response, your body will actually slow down fat burning—all for the sake of survival! When you stay in ketosis too long, the body starts to rebel, and conserve its fuel. A good analogy is if you lived out in the woods and you had stored 500 logs of firewood to heat your cabin for the winter. These were your reserves, because you could still go out and chop more firewood. So if you had to dip into these reserves, you wouldn’t be worried. But now it’s early March, and snow still covers the ground. It’s been a tough winter and you haven’t harvested many trees, and you see that you’ve got only 50 logs left. That’s a very low supply! Suddenly you’re reluctant to use them because you don’t want to run out. When you had 500 logs, you didn’t care about burning them. The same thing happens with your body fat. This is why when you stay low carbohydrate for too long, your body will slow down fat metabolism. And you know what? This is a good thing. There is a big difference between losing excess fat and losing all your fat. You want to lose excess fat. But your body needs a small amount of fat, and your primal brain is smart to insist that you meet this minimal requirement. Thyroid Problems Hormone production is often a function of a “chain of command,” as orders are passed from one organ to another, until the desired outcome is achieved. The hypothalamus and the pituitary gland, which are located in the brain, help control the thyroid gland, a butterfly-shaped gland located at the base of your throat, just below the Adam’s apple. To provide their direction to the thyroid, the hypothalamus releases thyrotropin-releasing hormone (TRH), which stimulates the pituitary gland to release thyroid-stimulating hormone (TSH). When the hypothalamus and pituitary are working normally, they respond when: Thyroid hormone levels are low, so they secrete more TRH and TSH, which stimulates the thyroid to make more hormones. Thyroid hormone levels are too high, so they secrete less TRH and TSH, which reduces hormone production by the thyroid. The thyroid gland uses iodine from food to make two thyroid hormones: triiodothyronine (T3) and thyroxine (T4). The primary secretory product is inactive thyroxine (T4), which is a prohormone of triiodothyronine (T3). T4 is converted to T3 peripherally by tissues with high blood flow, including the liver and kidneys. In the brain, T4 is converted to active T3 by type 2 deiodinase produced by glial cells. The thyroid stores T3 and T4, and releases them as they are needed. These two thyroid hormones function in almost every cell in your body, and play a fundamental role in regulating your metabolism and maintaining your resting energy expenditure—that is, the energy you burn just to keep functioning while sitting on the sofa. Diet can influence thyroid hormone levels, and prolonged ketosis and severe calorie restriction can lead to reduced thyroid function, or hypothyroidism, which is a mechanism to ensure survival in response to perceived famine. The goal is to slow metabolism, preserve energy, and preserve vital organs. T4 is the inactive form of thyroid, it needs to be converted to T3 which is the active form used by the cells. Do you know which hormone helps with this conversion? Insulin. Low thyroid hormones cause a drop in levels of insulin, the hormone needed to transport sugar from the blood to different cells throughout the body. Chronic low insulin levels slow down cellular function, including muscle contractions and basic brain function. Over longer periods of time, low insulin levels compromise this T4 to T3 thyroid hormone conversion. Testosterone Deficiency Sex hormone binding globulin (SHBG) is a protein made by your liver. It binds tightly to three sex hormones found in both men and women—estrogen, dihydrotestosterone (DHT), and testosterone. SHBG carries these three hormones throughout your blood. SHBG controls the amount of testosterone that your body tissues can use. Too little testosterone in men and too much testosterone in women can cause problems. Factors such as sex, age, obesity, liver disease, and hypothyroidism can change the level of SHBG in your blood. Hypothyroidism is when your thyroid produces an insufficient amount of thyroid hormones, which can happen with prolonged ketosis. What I’ve seen reviewing lab work over the years is too much SHBG leads to lower testosterone levels, and not enough SHBG leads to an increase in estrogen; both can be problematic for men and women. Long term ketosis can lead to either of these. Leptin Reset Leptin is your “stop eating!” hormone that plays an instrumental role in your hunger and weight management. It comes from the Greek word ”leptos,” meaning “thin.” Fat cells produce leptin in proportion to levels of body fat: the more fat cells you have, the more leptin they make. When your body fat increases, your leptin levels go up. Your brain senses the increase in leptin, and thinks, “Okay, we’ve got enough fat in reserve, so we can stop eating.” Thus, you eat less and burn more. Conversely, when you don’t eat, your body fat decreases, leading your leptin levels to drop. Your brain, sensing a shortage of leptin, thinks, “Hmm, we’re low on fat reserves. Better find some food!” At that point, you eat more and burn less. This negative feedback loop is similar to the control mechanisms for many physiological functions, such as body temperature, breathing, and blood pressure. A properly working leptin system tells your brain that you have enough energy stored in your fat cells to engage in normal, everyday metabolic processes. It leads to better brain function, memory, metabolic performance, mental sharpness, and coordination, and it can even affect the regulation of mood and emotion. This is wonderful—except that many overweight people suffer from leptin resistance. They have plenty of blood leptin, but the brain doesn’t recognize it. Despite the abundance of leptin, the brain thinks there is too little leptin—and orders the body to keep eating! This is obviously a bad situation. What can cause it? Several potential mechanisms behind leptin resistance have been identified. Free fatty acids. Having elevated free fatty acids in your bloodstream may increase fat metabolites in your brain, and interfere with leptin signaling. Inflammation. Inflammatory signaling in your hypothalamus could be a cause of leptin resistance. Having consistently high leptin. It may be that your brain gets accustomed to a constant flood of leptin churned out by your fat cells, and becomes “numb” to it. So, what happens when you follow a low-carb keto diet that puts you in long-term ketosis? Perhaps paradoxically, many people do not suffer from an increase in appetite, as you might expect from falling levels of leptin. It’s complicated, but the short version of the story is that leptin levels will drop, which would normally signal your brain to start eating. But something else happens—your brain becomes more sensitive to the effects of leptin, so a smaller amount of leptin packs more punch. In addition, the brain’s response to leptin is inhibited by inflammation, resulting in increased leptin resistance. Obesity increases inflammation, which increase the brain’s “numbness” to high levels of leptin. Inflammation is dramatically reduced by sustained nutritional ketosis. The reduction in leptin resistance due to reduced inflammation adds to the brain’s heightened sensitivity to leptin. In other words, on a well-formulated ketogenic diet the brain perceives a greater satiety response to less leptin. This explains the paradox as to why we see a decrease in appetite with nutritional ketosis. In order to achieve a sustained state of ketosis, you need to occasionally boost your levels of leptin and send the signal to your brain that higher levels of leptin are normal, and that you need to keep eating. Don’t let eating get you down. Once you get back from an event, then focus on your normal routine. After a trip, get back to your fasting and get back to your keto diet; you're going to be fine. Having a negative relationship with food and your body is just not good for your mind, body, and soul. [29:40] Why Logan Loves Keto Grocery Shopping These Days When you go to the grocery store now, there is keto everything. In 2012, there was nothing keto. So, Logan had to get creative in the kitchen. If people think they can’t do keto because they love ice cream, that’s simply not true! There is keto everything in the grocery store. Always look at the ingredients and make sure that it actually fits into your lifestyle. [32:55] Having A Community On Your Keto Journey - What You Need To Know When Logan first started doing keto, he got a lot of negativity from trainers in the gym. Logan isn’t a science guy, he doesn’t know how things work, but he has his experiences. Always try things; you don’t have to keep doing it if it doesn’t work out. When Logan found the keto community, he felt so supported. The keto space is the best community because people want to work together as a team to help other people.
