Remember back in episodes 3 and 5, sometimes we do this because a patient is either doing well and no longer needs our care or maybe co-management with another provider is the next step. In this episode, we talk about the “not-so-fun” reasons. Maybe your treatment options aren’t working, so now we need to try something else? Maybe the patient has a condition outside of your scope or something potentially life threatening that needs to be referred out immediately?
