Mechanism of Androgen Deprivation Therapy
Androgen deprivation therapy (ADT) is the backbone of prostate cancer treatment, with options including GnRH agonists and antagonists. GnRH agonists act on GnRH receptors in the pituitary gland, initially increasing production of testosterone before downregulating the receptors and lowering testosterone levels. In contrast, GnRH antagonists immediately block the receptors, reducing FSH, LH, and testosterone levels. The choice between agonists and antagonists is often determined by insurance coverage and convenience. For patients with pre-existing cardiovascular conditions, GnRH antagonists may be preferred due to lower rates of major cardiovascular events.
Managing Side Effects of Androgen Deprivation Therapy
Patients receiving androgen deprivation therapy (ADT) should be counseled on managing side effects. Common side effects include hot flashes, bone loss, and fatigue. Non-pharmacological strategies like keeping cool, dressing in layers, and avoiding triggers can help with hot flashes. Calcium and vitamin D supplementation is important to counteract bone loss. Patients should also focus on long-term cardiovascular health and adopt a healthy diet and regular exercise routine. In cases where side effects are severe, pharmacological options like venlafaxine can be considered to manage hot flashes.
Choosing Hormonal Agents in Prostate Cancer Treatment
Treatment decisions for prostate cancer often involve selecting between different hormonal agents. GNRH agonists, including luprolide and goserelin, are equally effective, and the choice often depends on formulary and insurance coverage. GNRH antagonists like relugolix are less commonly used due to higher cost, monthly injections, and injection site reactions. Insurance approval and cost considerations can also influence the selection among oral options. Additionally, patients with pre-existing cardiovascular conditions may benefit from choosing the antagonist relugolix, as it has shown a lower risk of major adverse cardiovascular events compared to luprolide.
Other Treatment Options in Prostate Cancer
In addition to hormonal therapy, other treatment options are available for prostate cancer. These include immunotherapy (sipuleucel-T) and radiopharmaceutical agents (radium-223 and lutetium-177). Sipuleucel-T is an autologous cancer vaccine used in metastatic castration-resistant prostate cancer. It stimulates an immune response to attack prostate cancer cells. Radium-223 is a calcium mimetic that targets bone metastases, while lutetium-177 targets PSMA-expressing prostate cancer cells. Bisphosphonates or denosumab may be prescribed for patients with osteoporosis or osteopenia resulting from hormone therapy.
