Aug 21, 2023
auto_awesome
Dr. Jerry Zimmerman, Professor of Pediatrics, discusses the physiologic rationale and clinical evidence for using corticosteroids in septic shock. They explore the potential benefits and adverse effects of corticosteroids in conditions like COVID-19 and ARDS. They also discuss the use of oxandrolone for muscle protein accretion and the future of clinical research. Additionally, they cover the use of corticosteroids in adrenal insufficiency, ACTH stimulation tests, and optimal cortisol cutoffs.
The decision to prescribe corticosteroids in critically ill patients depends on various factors like patient history and severity of condition, and appropriate dosage can be determined through serum calcium check and random cortisol test.
Different clinical scenarios require different indications for administering corticosteroids, such as primary adrenal insufficiency, chronic steroid use, and specific conditions like COVID-19 and ARDS, highlighting the need for further research and platform trials in pediatric sepsis.
Deep dives
The use of corticosteroids in critically ill patients
The podcast episode discusses the use of corticosteroids in critically ill patients, specifically focusing on the dose and indications for administering hydrocortisone. The speaker explains that the decision to prescribe corticosteroids depends on various factors such as the patient's history, the presence of comorbidities, and the severity of the patient's condition. They emphasize the importance of checking serum calcium, considering the patient's history, and conducting a random cortisol test to determine the appropriate dosage. The speaker also highlights the need to balance the benefits of corticosteroids with their potential adverse effects, highlighting the complexities of monitoring side effects and optimizing patient outcomes.
Nuances in prescribing corticosteroids in different clinical scenarios
The podcast delves into the nuances and considerations when prescribing corticosteroids in different clinical scenarios. The speaker discusses the indications for administering corticosteroids in various conditions, such as primary adrenal insufficiency, chronic steroid use, and patients receiving accommodate. They also touch upon the use of methylprednisolone or dexamethasone for specific indications like COVID-19 and ARDS. Additionally, the speaker explores the potential benefits of including fludrocortisone and oxandrolone in treatment protocols and the need for further research and platform trials to study the use of corticosteroids in pediatric sepsis.
The importance of research and future directions
The podcast episode emphasizes the significance of ongoing research and future directions in understanding the use of corticosteroids in critical care. The speaker highlights the SHIPS study, which aims to investigate the effects of stress dose hydrocortisone on mortality and quality of life in pediatric septic shock. They discuss the need for additional sites to increase enrollment and underscore the importance of funding and collaboration for successful clinical trials. The speaker also mentions the potential of big data and the utilization of gene expression and endotyping tools to personalize corticosteroid therapy and optimize outcomes in critically ill patients.
Jerry Zimmerman, M.D, PhD, FCCM is a Professor of Pediatrics at the University of Washington and the former Chief of the Division of Critical Care Medicine and the Director of the Pediatric Intensive Care Unit at Seattle Children’s Hospital. He is a past president of the Society of Critical Care Medicine. Dr. Zimmerman is the co-editor of the textbook Pediatric Critical Care and is an accomplished researcher. He was a charter principal investigator in the Pediatric Acute Lung Injury and Sepsis Investigators (PALISI) network and is a co-principal investigator for the Stress Hydrocortisone in Pediatric Septic Shock (SHIPSS) trial that we will discuss later in this episode.
Learning Objectives:
