Cystitis vs Pyelonephritis in Catheters
- Lower urinary tract infections like cystitis rarely cause fever or sepsis in immunocompetent patients.
- Catheter-associated urinary infections mainly become concerning when infection ascends to the kidneys causing pyelonephritis.
Bacteriuria Often Colonization
- Bacteriuria in catheterized patients is very common but often represents colonization rather than true infection.
- Evidence shows no difference in fever or urinary symptoms between patients with or without bacteriuria.
Strict IDSA Diagnostic Criteria
- Diagnose catheter-associated urinary tract infection (CAUTI) only if all four IDSA criteria are met.
- Criteria: 1000 CFU bacteria, urinary symptoms, no alternative infection source, catheter present or removed within 48h.
