Oral Facial Pain: A Recognized Specialty
Oral Facial Pain is now recognized as the 12th specialty in dentistry, focusing on the diagnosis, management, and treatment of pain disorders of the jaw, mouth, face, head, and neck. The American Academy of Oral Facial Pain provides resources and support to both specialists and general dentists, including continuing education, publications, networking opportunities, and an annual meeting.
The Path to Recognition
The journey towards recognition as a specialty was a long and challenging process for Oral Facial Pain. The history of the field involves various paradigms and approaches, including TMJ-focused perspectives, psychological considerations, and medication-based treatments. The official designation as a specialty was received on March 31, 2020, during a time of uncertainty with the COVID-19 pandemic. Despite the challenges, the recognition opens new opportunities and allows the field to define its scope of practice.
The Appeal of Oral Facial Pain
Oral Facial Pain is an appealing specialty for dentists seeking new opportunities. It offers a different approach to dentistry, focusing on chronic pain patients that other dentists may not feel comfortable treating. The field provides a unique and rewarding experience, with the potential for a boutique fee-for-service practice and opportunities for professional growth, such as education, research, and teaching. Oral Facial Pain specialists have the chance to make a significant difference in patients' lives and address a growing need in healthcare.
Orthotics and the Comprehensive Approach to Treatment
Orthotics play a central role in treating oral facial pain. While night guards and mouth guards provide protection, orthotics are specifically designed to stabilize and position the jaw in a therapeutic way. They address issues related to clenching and offer longer-lasting effects. While general mouth guards can be effective for 70% of those with TMJ discomfort, orthotics provide a more intentional and sophisticated solution. Different types of orthotics can be tailored to individual needs, allowing for customization and improved outcomes. While other treatments like injections and medication are also valuable, orthotics remain a prominent and effective component of treatment.
The Fee-for-Service Model and Creating Value
This podcast episode discusses the importance of the fee-for-service model in oral facial pain practices. In this model, patients pay upfront for the treatment they receive, aligning with medical and dental care costs. While fees might seem high, they are reasonable considering the complexity of the treatments offered. The speaker emphasizes the importance of over-delivering value to patients, ensuring they are enthusiastic about the treatment. The practice aims to treat patients who genuinely want to be treated, avoiding arm-twisting or pushing treatment on those who are uncertain. By setting appropriate fees and providing exceptional care, practitioners can ensure both their own financial stability and the best outcomes for their patients.
Dr. Jeffrey Goldberg lives in Williamsville, NY, with his wife Melissa and their two daughters. After several years in practice, Dr. Goldberg returned to the dental school to complete a three-year residency and master’s degree in TMJ disorders and orofacial pain, and now serves as a Clinical Assistant Professor in this program.
