Cognitive Biases in Organizational Settings

One of the biggest things I see is opportunity cost neglect. We don't tend to take into account gains that might be associated with another course of action in comparison to the thing we're already doing. People will have underperformers in a job and focus on what would happen if they were let go, rather than looking for someone who's good at their new role.
