Is There a Tender Muscle in the Foot?
The targeted neurological exam is going to be what's quite important. Reflexes, clonus, particularly at the ankle and higher up, if you think something higher up could be going on. The history would inform your suspicion of other organ system involvement,. But maneuvers that don't substantially alter the likelihood of serious systemic disease or neurological compromise that needs surgical intervention are not super diagnostically helpful.
