DevOps - Root Cause Analysis

Donne John: I think there's a whole, we could turn this into the Donne John show. We've been doing this before DevOps was a term. Our experiences go back to 1980s working in large corporations. And so when I first heard this sort of counterculture root cause, it didn't sit well with me. But I did my homework and I had some great advisors. So why don't you go ahead and flesh out your thoughts about this subject; then I'll tell you what I've learned.
