The mind is physiologically altered. Suicidality is definitely one of the diagnostic criteria for a diagnosis of major depressive disorder. Some individuals don't generally complain about it, but they just are not happy. And you know, sometimes people can become functional in their dysfunction. Those are the ones that don't want to do anything about it.
