Leading and Lagging Indicators of Safety
Leading and lagging indicators of safety is absolutely not representative of interactions in a complex system. Measurements of safety activity rather than actual physical work environment is not the most useful representation. If you are trying to get people to do inductions, then measuring how many inductions people actually do is a good sign of whether you've successfully rolled out that practice. It's not an indicator of safety, it's an indicator of when the organization is doing what you're trying to get them to do.
