How to Avoid Failure in Innovation

If you fail, certainly you're going to learn something from it. But why wouldn't you try to avoid failing? I don't believe that innovation has to include failing. You can't turn innovation from an art to a science. There's just too many unknowns and variables that can't be controlled. And this is the thinking I've been through for the last 30 years.
