In both cases, I think you really want to treat predominantly with antibiotics. The whole issue of putting chest tubes into patients with cirrhosis is a little bit of a sticky wicket. You can put in a chest tube, but it's not a beautiful thing. They may lack local symptoms and signs of abdominal complaints. Just do the tap and inoculate those blood cultures at the bedside. It'll make you a better person.
