There's a gap, as you say, between knowing what we're supposed to do and doing it. How do you bredach that gap? I don't have perfect answer for that. The ultimate solution is essentially environmental, the architecture of your environment. Don't rely on people to make those kinds of decisions. Stop making individual decisions and start using processes and environmental changes.
