The Non-Inferiority of High Flow in the Step Up Trial

In the step down post-exturbation trial, we could not demonstrate the non-inferiority of high flow. The patients who were switched from high flow to CPAP for patient clinical deterioration was actually quite similar in step up and step down. It's the discomfort that seems to be the big driver for switching patients from CPAP to high flow. So I think there are clear differences in the populations and this justifies, I suppose, taking a step back why we did this trial in two different populations differently rather than merging them all into one trial. But there are limitations that need to be mentioned and maybe we'll discuss many of them.
