The carbs scale up or down with skeletal muscle utilization of carb. Anyone who's severely ins resistant or diabetic has way less skeletal muscle than the should. They're probably not exercising optimaly, and they have muscles that are filled with fat and glycgen already. So their glucos disposal in their muscles is going to be poor.
