The most important first step is really establishing the correct diagnosis of DKA. A lot of time, I see one of the pitfalls in like interns or medical residents is when they see a patient with a low pH and hyperglycemia. So how do we do that? We would check a blood gas so that pH needs to be at least less than 7.3. The serum barcode will be low so that barcode needs to be less than 18. Either they need to have a positive urine or serum ketone level and also this patient will have NINGAP after doses. These are the objective lab that I will look for before making the actual diagnosis for DKA.
