Treating PF4 Anticoagulant in Patients With Hit

Pearl: In general, if you're concerned enough to be sending the PF4 antibody, you should be considering treatment. Treatment is at least discontinuing heparin, but also starting in non-heparin anticoagulant. Remember that inhibition of protein C by warfarin can cause some badness in patients with hit. If there isn't a thrombosis, then just treat until the platelets recover. Those with a history of hit should probably avoid heparin in the future.
