The Cuban Missile Crisis
The Cuban missile crisis is a good case study in near misses and points of failure. Kennedy was nervous that this would spiral into a nuclear war, but he actually didn't take the advice of his advisors. The Soviet archives opened up after the fall of the Berlin Wall and historians scoured newly released documents. What we learned was that the crisis was actually worse than we had thought.
