#35 – Oral Beta-Lactams, Are We Serious?

Breakpoints

CHAPTER

The Role of Dosing Intensity in the Treatment of Gram Negative Bloodstream Infection

For some oral Betelach Tam's like cepyroxine and cephedinear, we're actually unlikely to hit adequate exposures for bactoramia based on the usual dosing regimens. And in a study by Kotobbing colleagues that found an increased risk for infection recurrence with low bioavailability antibiotics, almost all of the patients that received those quote unquote low bio availability antibiotics received a suboptimal dose. So maybe we should be pushing the dose on some of these agents if we want to use them for more serious infections or for bloodstream infections. But there was no significant difference compared to fluoroquinolones as step down therapy.
