The most common cause of hepatic encephalopathy is non-adherence to lactulose or effaxment. The other four categories that we should think about are infection, increased generation of ammonia and decreased clearance of ammonia. Electrolyte disturbances such as hypokalemia can definitely increase ammonia levels and precipitate HE.
