How Do You Dose Opioid Oral Pain?

When you're starting somebody on oral pain meds, how frequently would you dose it? I see this so often in the hospital. C, four c, six q, eight q, 12 hours. How do you approach it of determining the optimal frequency for acute pain and particularly somebody who's got really severe pain like this? M gentlemen, i would likely dose whatever i'm giving, whether it's hydromorphone or oxicoton only.
