Nuclear War
The systems and structures we have in place create opportunities for failure. I feel pretty confident that we can't just keep rolling the dice and expect to always avoid nuclear war. We need systems that fail safe all the time. The risk of miscalculation or technical error, the fact that we could create a civilization threatening catastrophe because of that is so foolish.
