Learning Management System - What's the Logistical Model?

Every recommendation I've ever seen, these forms are filled out. What's the system then to track what that looks like inside of a learning management system? That's a good question. I am not a huge fan of a lot of learning management systems. But for myself, recording it in my own spreadsheet works pretty well.
