Do You Want to Give Good or Bad Feedback?

A manager says you usually have good and bad feedback that you need to convey back to your direct. How do you position it? Do you start with the positives and then move on to the negative? Are there any particular scenarios where documentation is recommended? Or should we just bypass and not document the one-on-one conversations?
