The system gets tricky when you have older people who have more than one underlying condition. Only around ten percent of people get medical autopsies, most of the time it's like, ok, this person's 85 years old and they went to sleep and they just kind of never woke up. Its kind of a judgment call what gets written on the death certificate as an underling cause of death.
