We've created too much fear in speaking up when in fact there's so much value in avoiding harm. I think a lot of us grew up in an educational system where you wanted to have the goods before you raised your hand in class. It'd be nice to speak up and say something smart, but the last thing you want to do is raise your hand and say something that ends up being criticized or is wrong. So we'd like all of us to think about whether or not we should be speaking up more often in avoiding errors of omission.
