Ep.85 Why does safety get harder as systems get safer?
The Safety of Work
Risk Risk Management in a Dynamic Society
The context and system drive the behaviour, as sydney decker would say. And we don't have conscious control over which mode of thinking we're doing or how our brains are optimizing what it's doing and not doing. We just do it. Sometimes errors come out of that process, and they're just a natural thing that then feedback to speed up or slow down in order to cope with them. The way mabertie puts it is he says, fundamentally, an operator does not regulate the risk of error. He s a high performance objective at the lowest possible execution cost. In the human mind, error is a necessary component of this optimized performance result. Amilberdy
