What Could Have Happened to the Pilot?
This plane had not been updated yet, despite the that this service bulletin had come out almost a year before this accident. The first officer inadvertently triggered the go lever. It is considered unclear descriptions of the automatic flight system in the flight crew operating manual prepared by the aircraft manufacturer contributed to this. Captain's judgment of the flight situation, while continuing approach, was inadequate and appropriate actions were not taken. We'll post a picture on sopomedia.
