Beta-Lattams: A Reasonable Step-Down Therapy for Bacteria

A recent study in JAMA looked at this very question across 114 VA hospitals, including just over 4,000 patients. About a quarter of them received beta-lattam step-down therapy after one to five days of parenteral antibiotics for racteremia due to enterobacteralis. And the remaining received of fluoroquinolone. Ultimately, what you reported was a combination of 30-day all-cosmortality and 30-day recurrent bacteremia,. which occurred at a rate of 4.4% in the beta- lattam group versus 3%.
