How to Discharge a Patient on Anticoagulation

If you've done a 24-hour trial of anticoagulation and their symptoms are not better or they're still incredibly breathless, do it very carefully. Those are the kind of patients to get a little bit more aggressive. I did want to ask about the PERT team. How do you decide what to just not treat? Isolated subsegmental pulmonary embolism is really tough. We probably don't have enough evidence right now to guide us strongly in either direction.
