The Different Types of Motor Neuron Problems
Dr. Dewey: The motor exam can be very nuanced, but generally you want to make sure that you're doing your observation, your palpation, and your strength testing. You should not have positive pathologic reflexes, such as the bibinski or Hoffman in the lower motor neuron injury. An intracranial lesion causing weakness is going to cause a contralateral hemiparisis or phlegia. Weakness can be due to sudden or rapid demalination, such as in multiple sclerosis or PML. Can progress very rapidly, even over hours, and some patients experience it as a paroxysmal onset or sudden onset of symptoms.
