Just culture is not essential for learning, because people will learn under any circumstances irrespective of the culture that's in place. Learning from mistakes versus blame individuals only focuses on how an organization deals with things that go wrong. The other four key components are adopting a model of shared accountability, managing behaviorr choices and designing a designing safety into all you your systems and processes.
